Training Officer/ Case Management

JOB DESCRIPTION SCOPE OF WORK: The CP training officer-Case Management focal point will be responsible to manage the overall Case Management program in collaboration with training department. The training officer- Case Management focal point will be responsible of the supervision and support for the social workers.     Position objectives: Coordinate with  the case management team and ensure that their work is according to standards Provide technical support to the case management team Coordinate with Training manager and program manager and program manager assistants Who this position reports to:  Training Manager   RESPONSIBILITIES: Program Ensure that the Case Management system is functional and follow the SOP. Ensure that staff fill in correctly the referrals and other tools Provide feedback to the training Manager and the Program manager and assistants in terms of needs for capacity building  Support the social workers in their daily work ·Organize the case conference, to discuss challenging cases, ensure timely follow up, and that all systems are functional facilitate  CM trainings and coaching to TDH staff and partners Participate in the development of lesson learned and best practices Participate in case closure meetings with the social workers Monitoring & Reporting Develop weekly monitoring plan for the case worker Participate in the monthly report with the training department team Report the monthly new cases to the M&E department and program manager Report the monthly closed to the M&E department and program manager   Coordination & Representation Attend CP coordination meeting when required Liaise and collaborate with other CM actors when required Maintain positive coordination and relationships with partner and other TDH sector staff. Other Ensure activities are done in accordance withTDH logistics policy and in a timely manner to ensure the successful implementation of programs; Other duties as assigned by the supervisor to enable and develop TDH programs
