Medical Department Paediatric Advisor
Main Purpose
Organize & carry out anaesthesia activities for paediatric patients under 15 years of age at the Elias El Hrawi Governmental Hospital Surgery Unit (pre-anaesthesia evaluation, administration of anaesthesia, recovery from anaesthesia, anaesthesia pharmacy, equipment and general service organization, etc.), according to MSF policies, protocols and universal hygiene standards, in collaboration with the pediatric surgeon and other medical staff, to ensure the quality of care provided to pediatrics patients.
Accountabilities
Carry out pre-operative consultations by evaluating the risks of surgical interventions and the presence of coexisting disease, providing the patient and his/her family or care takers with appropriate information about the risks and benefits of the type of anesthesia to be used, and obtaining their written informed consent, in order to ensure the best conditions for surgery.
Carry out age-appropriate anesthesia care for patients under 15 years of age and related activities to ensure safe and adequate anesthesia during the procedure:
Prepare beforehand the drugs, material, and equipment required.
Administer and maintain anesthesia following the established rules of hygiene and asepsis in force.
Monitor patients throughout the intervention.
Ensure, respect, and promote medical confidentiality of patients and their families.
Ensure the safe transfer of patients to the recovery room, verifying the full recovery of the patient’s consciousness and respiratory/hemodynamic stability, ordering appropriate of plan of care for analgesia, nausea & vomiting treatment/prophylaxis & fluid management, and providing a complete handover to the nursing/medical staff. Ensure that appropriate criteria are met prior to discharging the patient from the recovery room.
Participate in clinical rounds with the pediatric surgeon once or twice a day, discussing treatment plan with the appropriate members of the medical team, renewing orders and ensuring they are carried out, and systematically evaluating the patient’s level of pain in order to modify the analgesia plan and/or therapies for their relief.
Carry out administrative procedures and documentation (timely and legible charting in the patient medical record, completion of forms, statistics, data base entry, etc.), in order to have updated and accurate information about the anesthesia care provided and the day-to-day activities of the project.
Advise, as needed or requested, the pediatric surgeon and other staff on postoperative care in the wards concerning fluid management, oxygenation and airway maintenance, cardiovascular homeostasis, chest physiotherapy and mobilization, with special focus on patient pain relief during the entire hospitalization.
Adhere to all hygiene rules, procedures and MSF protocols in OT/recovery with regard to anesthesia material (decontamination, sterilization, cold chain and storage conditions, etc.) to ensure the safety and protection of patients and staff. Know accidental blood exposure policy and procedure and implement them when necessary. Never reuse syringes between patients under any circumstances.
Ensure good functioning and maintenance of the anesthesia equipment (oxygen extractor, anesthesia machine, equipment, etc.).
Organize the tasks of personnel under his/her responsibility.
Respond to requests for anesthesia support in the pediatrics wards and emergency department, intervening when needed, and participate in any emergency activity.
In collaboration with the pediatric surgeon, carry out quantitative surgical-anesthesia data collection, complete and submit reports for the monthly follow-up of perioperative mortality, and for the quality of anesthesia/pain management, and inform immediately the direct supervisor in the event of medical error. Participates in quality improvement activities (morbidity and mortality reviews).
