Case Manager

Main Objectives: To assess, develop pathways for and report on specific CP cases. To liaise between various actors involved in the response and function as a link across various sectors. To assess specific cases of child protection, develop a response plan and ensure that planned steps are followed up on. Responsible of triggering needed coordination mechanisms with actors and sectors involved in the response.   Main Responsabilities: Develop, update, communicate regularly the SOP, referral pathways, information sharing protocol for Case Management. Lead the Case Management Meetings and guide the development of a support plan for targeted children, possibly together with them and their families Lead the Case Conferences and liaise with other participants in order to share and compare responses, prioritize cases, and design joint responses Keep records of cases managed and mentor staff involved in the process Support Protection Assistants with family visits and assessments In collaboration with the CP Officers and CP Specialist, develop capacity plan for the CP Assistants Report back to M&E team on case management activities and support staff in charge of data compilation Work closely with the Psychologists, Speech Therapist and Ortophonists when needed Attend CP or Education coordination meetings when required Liaise and collaborate with other CM actors Perform any other relevant duties as required
