Patient Therapeutic Education (Pec) In Cncd Activities - Tripoli

Scope of responsibility: Under the responsibility of the medical team leader and the Health education supervisor PTE performs comprehensive counselling and education to CNCD patients in order to maximize adherence to the medical treatment, and thus improving the quality care and autonomy of the patients Activities: Provides individual support to the patient, in the process of diagnosis and treatment of CNCD through “Therapeutic educational sessions”, “Counselling” and  “Orientation/evaluation” Organizes health educational sessions at the clinics, for CNCD patients, providing therapeutic education and empowerment of the patients through group discussions In close collaboration with HE supervisor, provides continuous training and group discussion with the CNCD team Organizes with the CNCD team case study meetings to reinforce the therapeutic education approach Active Provides a detailed monthly report of activities to HE supervisor Implementation of a filing system for CNCD patients in the PTE component Assures an adequate communication with the CNCD team to provide a continuous feedback of the patients in PTE. Provides therapeutic education for other programs (ex. SRH, MH,PHC), upon request from medical team Participates in the creation of education therapeutic tools with the HP team Support the HP team on the medical part of the awareness sessions   Note : This Position to Cover Maternity ( 4 months)
