Case Manager

• Develop, update, communicate regularly the SOP, referral pathways, information sharing protocol for Case Management • Lead the Case Management Meetings and guide the development of a support plan for targeted children, possibly together with them and their families • Lead the Case Conferences and liaise with other participants in order to share and compare responses, prioritize cases, and design joint responses • Keep records of cases managed and mentor staff involved in the process • Support Protection Assistants with family visits and assessments • In collaboration with the CP Officers and CP Specialist, develop capacity plan for the CP Assistants • Report back to M&E team on case management activities and support staff in charge of data compilation • Work closely with the Psychologists, Speech Therapist and Ortophonists when needed • Attend CP or Education coordination meetings when required • Liaise and collaborate with other CM actors • Perform any other relevant duties as required
