Medical Case Manager

The required tasks would be:   1-     Manage the hotline and refer refugees to adequate health services   2-      Orient refugees to available services based on the needs   3-      Take note of cases that need further assistance from UNCR/Caritas side and communicate properly   4-      Communicate reported problems/issues raised by refugees concerning health care services   5-      Refer cases to different partners based on the needs   6-      Receive calls of Community Based Health Workers (CBHWs) and provide needed info   7-      Refer cases to the CBHWs to follow-up   8-      Communicate with the Public Health Associate (PHA) in Tripoli Sub-office regarding complicated cases   9-      Follow-up on action taken by partners and report delays to the PHA   10-   Submit a weekly report to the PHA (in an excel sheet) reflecting cases referred and actions taken
