Essential Duties & Responsibilities:
Provide individual therapy to Child/Adolescent Mental Health Syrian Refugee population and vulnerable host population referred from within or from outside the center.
Complete assessments, management, evaluations, treatment recommendations, reviews, and other documentation as necessary.
Interface with families, treatment team members and case management as requested by IMC.
Complete the section related to psychotherapy in the care plan along with the case manager and psychiatrist.
Ensure follow up of the patient along with the case manager and provide constant recommendation based on the output of therapy sessions, home visits and outreach work.
Keep track of patient numbers/cases through IMC outpatient records, to be submitted at the end of every month.
Document the process of therapy in every patients file through progress notes to be submitted in a timely manner.
Make necessary and timely referrals when necessary.
Attend weekly or biweekly meetings including the technical meeting with the rest of the CM team.
Develop an exit strategy in coordination with the case management team at least 6 to 8 weeks prior to termination of therapy.
Attend the supervision meeting that is conducted once a month.
Report directly to IMC Area Manager.
Deliverables:
Submit outpatient records at the end of each month.
Submit timesheets of executed works that include number of sessions, beneficiaries/patients. This timesheet has to be signed and approved by the Psychotherapist and an IMC person (center manager, social worker….).
Fill in a monthly summary of MH diagnoses.
