Scope of responsibilities: Carry out all the activities linked to the social field in order to ease the patient situation and to maximize its adherence The Social Worker must ensure confidentiality in all his/her activities. Function responsibilities: Network Keep; maintain a network of social partners that can provide answers to social problems and medical care (institutions, NGOs…etc.) especially for displaced Syrians Establishes a proper referral system with other organizations for services unavailable at the MSF structures and coordinates and follows up the referral of MSF mental health and PHC patients with such partners. Liaise with these organizations as needed. Social activities Work as a member of a multidisciplinary team and ensure a good flow of communication of any relevant information. Identification of social, psychosocial, socio-economic and administrative needs among patients Provide information, advise on existing social services Provide immediate social care (actively listening, clothes…etc.) Refer patient and his/her social problems to the relevant person or institution (inside or outside MSF Monitoring, reporting Participate in the development of the plan of action Keep statistics updated about his/her activity nform his hierarchy when unstable situation (social, emergency, security) is seen, asessed or faced. Actively participate in clinical and medical meetings as well as community mental health events/activities. Immediately inform the medical team on any patient requiring medication education, hospitalization, emergency psychiatric assesment. Specific activities Linked to the context Specific to Mental Heath activities Involve the patient and the family in the development and implementation of individual action plans. Case manage the psychotic patients of the project Provide basic counselling-support to MSF beneficiaries Conduct home visits upon request to perform social assessment or psychoeducation or to cross match the mental health signs and symptoms in patient family environment, or to work on family dynamic Follows-up defaulters.Take part in the psychosocial assessment of families living in shelters and in the constitution of support groups
