PEDIATRICIAN

Main Purpose

Providing outpatient and/or inpatient paediatric care to patients/beneficiaries according to adapted and updated medical knowledge, MSF protocols, values and universal hygiene standards to improve the new-born and child/beneficiaries health conditions

Accountabilities

• Applying medical knowledge and skills to prevention, diagnosis and treatment of children, ensuring integrated new-born and pediatric care through Emergency, OPD, and IPD services in Newborns and children. Prescribing the necessary treatment, respecting MSF protocols.
• Collaborating closely with the different department of the hospital (e.g. Maternity), Knowing and ensuring all updated MSF medical protocols and universal precautions are followed and implemented, reducing bio-hazard risks and improving infection control.
• As an expert in pediatrics, proposing strategy (modification on current new-born and pediatric care management) and implementing them after validation of Operation and Pediatric Advisor.
• Ensuring on-going new-born and pediatric training (formal and bedside) of the medical/paramedical multidisciplinary team in order to optimize the new-born and pediatric quality of care. Reviewing and discussing selected complicated cases or mistakes and giving technical input.
• Keeping patients and their families informed of the illness and the treatment to follow, checking that they have been understood. Ensuring professional autonomy and confidentiality are respected and a good attitude and proper bedside manner towards the patient and their care takers and the rest of the medical team.
• Following up the evolution of the hospitalized/IPD patients through daily visits, consultations and examinations, prescribing the necessary treatment following MSF protocols, deciding whether they can be discharged or transferred to others hospitals – in collaboration with other doctors- and informing their family about the patient’s evolution.
• Monitoring the proper functioning of the department, equipment or material and addresses the list of the missing items for newborn and paediatric activities. Providing inputs about general organization of the different paediatric wards, including the admission, transfer and discharge criteria + referral procedure from Maternity and post-delivery wards
• Checking and controlling the rational distribution of medicines and equipment under his/her responsibility and taking care the quality, disinfection and sterilization of the medical material. Assuring general compliance with standing hygiene standards.
• Participating in the introduction / improvement of the collection and analysis of epidemiological newborn and paediatric data, checking its validity and informing the line manager or project coordinator about any problems or complications of the patient’s illness and medical errors.
• Managing the team under his/her direct responsibility according to MSF HR policies and procedures, supervising their performance, organizing and scheduling shifts and rotations, directly participating in emergencies and on calls if necessary.
• Designing and providing training to staff as required.
• Debriefing with coordination, Headquarter (including Pediatric Advisor) and providing written report on main findings, activities, conclusions and further recommendations.

MSF Section/Context Specific Accountabilities

Context

An approximate 500,000 Syrian refugees are currently present in the Beqaa valley. Beqaa hosts the largest concentration of Informal Settlements. Many refugees live there already since years in very difficult circumstances. The existing health structures are expensive, overloaded and often cannot cope with the increasing demands. Due to the huge number of refugees compared to the small country, Lebanon closed its borders to Syrian refugees in January 2015 and UNHCR is no longer registering any more refugees. The new, illegally entered refugees have no access to the public health care system at all and are therefore even more dependent on MSF health care.

Activities

MSF is providing comprehensive Primary Health Care for both Syrian refugees and vulnerable Lebanese through clinics in 5 locations in Arsal, Baalbek, Majdal Anjar, Barelias and Hermel. This includes consultation and treatment for non-communicable chronic diseases, SRH (incl. an SGBV component), health promotion and mental health services.

There is an extra focus on pregnant woman through the Mother and Child Centers (MCC) in Arsal and Majdal Anjar.

Paediatric consultations will be offered for 6 months in Arsal’s clinic.

Security constraints

The situation in Lebanon is complex and can be volatile. The Beqaa valley is prone to regular security incidents. Security rules are in place in the entire Beqaa valley; regular adaptation of security rules is constantly requested. With the removal of the last armed groups in Arsal, the expat team is now able to enter the town again after having managed the PHC and MCC via remote control since 2014.

Major stakes

a) Adaptation of MSF activities according to budget and main needs identified (closure of OPD general consultations, focusing on NCD, MH, and SRH)

b) Big project in terms of volume and activities: 5 clinics and 2 MCCs with large distances between each other and a wide variety of activities in each of them where strong collaboration is needed.

c) Increase in tensions, both sectarian and between the host communities and the refugees.

d) Reduction of other actors’ including INGOs’ funding and capabilities to respond to the crisis in areas with a high density of refugees and an increased vulnerability of the host population.

e) Due to Lebanese legal constraints, expat doctors are not allowed to consult patients themselves.

المسائلات:

• تطبيق المعارف الطبية والمهارات في الوقاية، التشخيص ومعالجة الأطفال، وتزويد رعاية متكاملة للمولودين الجدد والأطفال في قسم الطوارئ وقسمي المرضى الذين يتم استشفائهم والمرضى الذين لا يتم استشفائهم. بالإضافة الى وصف العلاج الضروري مع احترام بروتوكول أطباء بلا حدود.
• التعاون بشكل مباشر مع مختلف أقسام المستشفى (مثل قسم التوليد)، مع معرفة وضمان اتباع وتطبيق كافة بروتوكولات أطباء بلا حدود الطبية المحدثة ومعايير الوقاية العالمية، مع تخفيض المخاطر البيولوجية وتحسين نوعية ضبط العدوى.
• بصفة خبير في طب الأطفال، اقتراح استراتيجية (تعديل إدارة رعاية المولودين الجدد والأطفال الحالية) وتطبيقها بعد الحصول على الموافقة من الإدارة والمشرف. 
• التأكد من القيام بتدريبات (رسمية وطريقة معاملة الطبيب للمريض) حول طب المولودين الجدد والأطفال للفريق الطبي/المساعدين العاملين في مختلف المجالات من أجل تحسين نوعية الرعاية المقدمة للمولودين الجدد والأطفال. مراجعة ومناقشة الحالات المعقدة أو أية أخطاء وإعطاء الرأي التقني.
• إبلاغ المريض والأهل عن المرض والعلاج المعطى والتأكد من فهم المعلومات. ضمان احترام الاستقلالية والسرية المهنية والتمتع بسلوك جيد وطريقة ملائمة لمعاملته للمريض والأهل وبقية الفريق الطبي.

• متابعة المرضى الذين في المستشفى من خلال الزيارات اليومية، الاستشارات، الفحص، وصف العلاج المناسب حسب بروتوكول أطباء بلا حدود، أخذ الفرار اذا يجب تسريحهم أو تحويلهم إلى مستشفيات أخرىبالتعاون مع أطباء آخرين و إعلام عائلاتهم حول تطور حالة المريض
• مراقبة حسن سير عمل القسم، المعدات، أو المواد ، يضع قائمة بالأغراض غير الموجودة لنشاطات طب الأطفال و حديثي الولادة ، توفير المعلومات حول التنظيم العام لمختلف أجنحة الأطفال، و التي تتضمن معايير الدخول، التحويل، و التسريح + إجراءات التحويل من أجنحة الولادة و رعاية ما قبل الولادة
• التحقق والتحكم بالتوزيع المنطقي للأدوية و المعدات التي ضمن مسؤوليته\ها و الانتباه إلى نوعية، تطهير و تعقيم المواد الطبي. ضمان الامتثال لمعايير النظافة العامة
• المشاركة في تقديم / تحسين جمع و تحليل المعلومات الوبائية الخاصة بالأطفال و حديثي الولادة، التأكد من صحتها و إعلام المدير المباشر أو مدير المشروع عن أي مشاكل أو مضاعفات تخص مرض المريض و الأخطاء الطبية
• إدارة الفريق الموجود تحت إشرافه\ها حسب سياسة و إجراءات الموارد البشرية لأطباء بلا حدود، الإشراف على أدائهم ، تنظيم و جدولة المناوبات و المداورات، المشاركة مباشرة بحالات الطوارئ و جاهز للاستدعاء عند الضرورة
• تصميم و توفير التدريبات للموظفين حسب المطلوب
• استخلاص المعلومات مع مكتب التنسيق، المركز الرئيسي (من ضمنها مستشار طب الأطفال) و توفير تقارير مكتوبة عن أهم النتائج، النشاطات، الخلاصات، و التوصيات
