1. Mandatory Required Documents are as follow:

• Bidder's official registration documents
• Bidder must sign and stamp the invitation to bid
• Bidder must sign and stamp the sworn declaration
• Bidder must fill, sign and stamp the third party beneficiary information
• Bidder must fill, sign, and stamp the price offer

2. The Grading during the evaluation is defined as below:

• Experience (10 points)
• Price (70 points)
• Quality (10 points)
• Availability (10 points)

3. N.B:

• LebRelief can drop out the price offers if all the essential required documents are not submitted with the price offer.
• Invitation to bid, the Sworm declaration, third party beneficary information and price offer form should be signed and stamped.
• This call for tender will be conducted using LebRelief standard bidding documents, open to all qualified suppliers and service providers.
• The bids will be reviewed after the submission deadline in an online session and link will be shared at later stage.
