Clinical Case Manager

  1. Key Responsibilities
  1. Medical Case Management
  • Monitor and follow up on overall medical care plans for beneficiaries.
  • Work with social workers, and as needed, medical providers to consolidate medical treatment plans and communicate updates to the Executive Director, and the Medical Committee. This includes joining social workers for clinic appointments where needed (i.e second opinion solicitation, cases experiencing medical complications, cases with conflicting medical opinions…).
  • Meet regularly with social workers responsible for day-to-day patient coordination to ensure alignment of care and guide with respect to clinical care follow ups.
  • Identify care issues such as conflicting treatment plans, delays, complications, or extended treatment timelines and coordinate with primary provider for clarity with escalation to medical committee as needed.
  • Oversee the medical clearance process for patient enrollment.
  • Ensure compliance with organizational medical policies and standards.
  • Screen urgent medical complaints from patients and families to assess the need for emergency intervention or referral to one of the Fund’s designated physician focal points, in accordance with applicable policies and procedures.
  1. Medical Committee Coordination
  • Serve as a standing member of the Medical Committee
  • Present cases requiring committee review as per terms of reference of the medical committee
  • Maintain a structured feedback loop between social workers, rehabilitation specialists, and treating physicians.
  1. New Cases and Medical Coordination
  • Communicate with stakeholders to collect medical data and supporting documentation for new cases.
  • Oversee preparation of medical dossiers and required documentation, ensuring alignment with clinical needs and new case criteria.
  • Ensure adherence to patient enrollment policies and medical eligibility requirements.
  • Assess and coordinate medical support required for families during their stay.
  • Ensure that any modification or exception to medical guidelines is formally documented and approved by the appropriate authority
  1. Collaboration and Patient Support
  • Work closely with social workers and the fund’s multidisciplinary teams to ensure holistic, patient-centered care.
  • Coordinate with approved external organizations and service providers to secure additional medical or psychosocial support when needed.
  • Maintain effective communication between families, medical teams, administrative staff, and support units.
  1. Documentation and Reporting
  • Maintain accurate and up-to-date medical case files and reports as per fund requirements.
  • Track new case requests, approvals, and related documentation.
  • Prepare and submit regular updates on patient progress and arrival status.
  • Provide administrative support strictly within the scope of medical case coordination and refer non-medical requests to the appropriate department.
  1. Safeguarding, Ethics, and Compliance
  • Uphold the highest standards of medical ethics, patient confidentiality, data protection child protection, safeguarding, and prevention of sexual exploitation and abuse (PSEA) .
  • Ensure that all medical decisions remain under the authority of the designated medical team.
  • Promote adherence to safeguarding policies and humanitarian principles in all patient interactions.
