Hospital Referral Focal Point (03 positions) - 7 months project duration

  1. Key Responsibilities
  • Act as a primary focal point 24x7 (in rotation) for receiving individual health referrals for persons of concern from health sector and multisectoral partners.
  • Channel the cases received through the appropriate specialized health services and provide feedback to the referring organization as needed on the referral outcome.
  • Ensure close follow up on all managed cases, until access is ensured, to the extent feasible.
  • Keep track of cases referred in an online tracking sheet, that would allow relevant data extraction and make sure it is regularly updated.
  • Liaise with various stakeholders including hospitals, partners, Globemed and other relevant entities, as needed to ensure proper follow up on cases managed.
  • Advocate with the relevant stakeholders to ensure access.
  • Report to National Health sector coordinator or his/her designee on any issues to address the challenges faced by the vulnerable population
  • Ensure knowledge and understanding of the relevant referral pathways and SOPs.
  • Report non-conformities identified in the provision of health services at PHCs or hospitals.
  • Ensure confidentiality of the medical information of the vulnerable population
  • Report on weekly basis on the relevant activities, including figures of cases managed.
  • Participate in regular meeting with Health sector coordination team and other organizations supporting the secondary health care for the most vulnerable population
  1. Deliverables:
  • Referral system operational within the first month.
  • Case tracking database maintained and regularly updated.
  • Referrals processed and feedback provided in a timely manner.
  • Weekly and monthly reports produced with key insights.
  • Coordination and referral pathways strengthened for sustainability.
  1. Technical Supervision and Reporting

The staffs will be directly reporting to the Health sector/ cluster coordinator or his/her designee.

oEducational Requirement: First university degree in nursing, public health or any other relevant field, or relevant work experience in case management.
Experience Required: 1-3 years’ experience in a humanitarian context preferably in Lebanon.
Skills / Technical Skills and Knowledge:
oRequired: Familiarity with the Lebanese health system and MoPH structures is highly desirable.
oDesirable: Strong interpersonal communication, coordination, and analytical skills.
