Medair will provide the below forms.

2. Medair supplier registration form
3. Medair Code of Ethics
4. RFX-2026-0001185

Bidders to provide, their bids and documents as per the below specifications. Once completed all to be uploaded Into the MS office online form.

1. Supplier registration form: Filled, signed and stamped, supported with the following documents:

• The company legal registration documents and VAT certificate
• Company portfolio
• Proof of Company bank account in USD, approved from the Bank (Online bank account approval is acceptable)
• Bank statements for 1 year,
• Financial report / audited copy of Balance Sheet and Income Statement or your Annual report
• Experience in similar projects at similar scale, by providing reference letters and/or project completion certificates.

Medair Code of Ethics Please read sign and stamp

• RFX: Filled signed and stamped in PDF format and excel format
• Delivery lead time (please fill it in the RFX column “offered delivery date”)
• The lead time to start on site after the approval to start for please add the answer in the workplan.
• Machinery: (Availability and capacity of required machinery) and the legal registration documents for each machinery
• Insurance for each Machinery or one insurance for all.
• Payment terms (please fill it in the RFX “column payment terms”).

General Note

• Medair might decide during the evaluation to visit the supplier office.
• Medair will Sign Framework agreements with the suppliers/contractors, Medair might select more than one supplier/contractor for each activity.
• Subcontracting is not allowed and it will lead to breach of contract.
• Quantities provided are for quotation and framework agreement purposes. Orders will be based on assessed quantities.
