Nurse Referral

Main Purpose Expedite referrals of patients from MSF health structures to national hospitals, in line with MSF referral policy, including the follow up of referral cases to ensure the achievement of a quality service, under the supervision of the referral activity manager. Accountabilities Facilitate MSF referrals to secondary hospitals: ensure that MSF patients are referred according to the adequate criteria and that the documentation is adequately written. Contacting the admission office of the referral hospitals according to the priority list and MSF protocols. Be available for transfer of patients in ambulance, ensuring quality of care on transfer of patients in line with MSF standards & protocols. Daily checkup of the ambulance, to ensure all equipment, medications and materials are up to date and available. Report back to manager in case of any missing or expired items. Assist Patients experiencing difficulties in accessing referral hospitals, thru the fast track registration availble in hospitals for non-registered patients in UNHCR. Receive patient’s calls and assist in any problems faced by patients in concern with the referral system and inform your direct manager. Refer patients that do not benefit from MSF coverage to the social worker using the social worker referral form.  Maintain quality of data collection for patients referred to secondary health care, including indications, treatments, outcomes, etc, using the proper excel files. Maintain good communication and relations with the referral hospitals & their representatives, and report any concerns or problems directly to your Manger. Assist and support the Nurses in the IPD (In Patient department) according to MSF protocols and policies when possible:                  Task:                      ü  Be considerate and helpful with other staff during workload hour. ü  Assist in the admission and discharge of patient ( record history and findings in the observation chart, monitor  vital signs for both the mother and newborn, check for uterine involution, bleeding and ensure patient’s pass out urine. ü  Document all findings and report to Dr on duty for any anomaly both for mother and newborn before and after delivery. ü  Provide health education on hygiene, breastfeeding, vaccination, good nutrition. ü  Assist in the newborn care and resuscitation if needed. ü  Ensure a complete handover to her colleagues.
