Gynecologist

Main Purpose Perform gynecologic and obstetric procedures, implementing any other prior evaluation and treatment required, during or immediately after a woman has given birth in order to improve women’s reproductive health and to reduce pregnancy, delivery and neonatology risks. This will be done in accordance with MSF policies, protocols and universal hygiene standards and in multidisciplinary collaboration with other medical doctors and midwifes. Accountabilities Provide health care related to gynecology and obstetrics to patients, including prevention, diagnosis and treatment. Ensure the implementation of MSF clinical policies, tools, guidelines and protocols related to gynecology and obstetrics in order to minimize clinical risk and provide best reproductive practices. In collaboration with the rest of the team, carry out the collection of quantitative gynecologic data and ensure exhaustive use of management tools in order to improve gynecological results and share knowledge among other field specialists. Supervise the maintenance and running of gynecologic equipment and drug consumption, and give input for the regular and intermediate medical orders preparing new orders when required and ensuring their rational use; this will be done in cooperation with the pharmacy and logistic staff as well as project management. Ensure good communication inside the WHC team and with referral hospitals MSF Section/Context Specific Accountabilities Clinical responsibilities: Gynecologist will be consulted by the MW in case of any deviation from the normal; and in attendance of any gynecologic/obstetrical emergency in IPD or OPD ( i.e. pre eclampsia, shoulder dystocia, prolonged second stage, etc.) During the day (0800-1600), the gynecologist is in the OPD available in case of any emergency and for any problem presented by the MW. Between 1600 and 0800 the gynecologist is in IPD  as leader of the IPD team, working alongside midwives and nurses, taking responsibility for all patient care and outcomes. OPD: Gynecologist is  responsible for both ultrasound and medical consultations of pregnant and postpartum women in OPD, with proper documentation He/She is responsible for referral to tertiary hospitals/additional clinical tests/examinations  according to  MSF referral criteria (lab,ultrasound,complicated pregnancies) He/she is responsible  for  internal referrals to  in PHC using clear concise documentation/phone calls Gynecologist will cooperate with PHC doctors for management of certain conditions (e.g. chronic diseases)  based on project referral criteria The Gynecologist follows and manages all high risk pregnancies. He/she is responsible for referral of High risk/complicated pregnancies in collaboration with the Gynecologist Team Leader and according to the high risk referral SOP .  Any unusual referrals will be communicated directly to the referral team to help with the referral process He/she ensures complete documentation in patient files and registers, including filling out the appropriate forms when sending patients to PHC and when referring for complete handover of high risk patients to RHUH to ensure good communication to the referring doctors IPD: In collaboration with the midwife, he/she examines the patient and makes a plan documented in the patients ‘file and communicated to the midwife/nurse and patient. Rationale should be well explained. The gynecologist does frequent rounds with the midwife (i.e. 7AM, 2PM, 430PM and 10PM) to be updated on the current patients in IPD and management plans. He/she Ensures updated and complete documentation All management plans strictly based on MSF protocols and up-to-date evidence-based recommendations/guidelines provided by the Gynecologist Team Leader, ensure rational use of medications (no treatment beyond MSF protocols) The gynecologist does performs ultrasound according with MSF protocol and  documents the result  on the ultrasound form that will be attached to the patient file) Gynecologist conducts normal deliveries and assists midwife/nurse when needed – both clinical care and documentation Gynecologist attends all complicated deliveries Gynecologist performs MVA for first trimester missed or incomplete abortions according to MSF protocol He/she validates  emergency referrals based on MSF guidelines and WHC referral criteria. Training and capacity building: Gynecologist  assists the Gynecologist Team Leader with developing trainings based on needs in the department, case discussions and evidence based practices to improve the quality of care and consultations.     Participates in all training delivered to the department and makes an ongoing effort to maintain and update obstetrical/medical knowledge and skills Referrals: Gynecologist is involved in the final decision for referral for high risk pregnancies and is responsible for accurate documentation on the referral form and for phone handover of urgent cases to the referral doctor Gynecologist  attends the  Monday morning RHUH Obstetrical rounds monthly to  follow-up referred emergency cases Communication: Can understand the main ideas of complex text on both concrete and abstract topics, including technical discussions in his/her field of specialization i.e. case conferences to discuss complicated/challenging cases Can produce clear, detailed text on a wide range of subjects and explain a viewpoint on a topical issue giving the advantages and disadvantages of various options.
