The Nurse Coordinator is responsible for directing, organizing and supervising the work at the PHCC, and ensuring that effective care is being provided to the beneficiary
Duties
Provides nursing services and treatments
Provides case management by being the contact person for beneficiaries
Assesses the healthcare needs of patients and families and determines the need for consultations with other services and health-care providers
Provides basic education and information to beneficiaries and families
Provides awareness sessions to beneficiaries and families about health issues
Documents actions by completing forms, reports and patient records
Documents accurate and pertinent patient information in a timely manner, including maintaining a comprehensive health summary
Safeguards health of children by effecting school health programs
Assists the physicians in their consultations when needed
Communicates with secondary and tertiary providers to ensure the continuity of care
Completes referrals, lab requisitions and diagnostic requisitions
Adapts the work and services provided at the PHCC according to the ongoing projects
Controls the stock of the center and makes sure that equipment and other supplies are in stock
Delegates safely and appropriately and supervises team members
Plans and delivers teaching for staff, students, beneficiaries and families
Participates in training, orientation, competency assessment and education of new employees
Communicates clearly, concisely and accurately, creating a working climate that provides growth and job satisfaction of personnel
Acts in a professional manner, reflecting the mission of Caritas Primary health care Center and demonstrates compliance with all policy and procedures
Collaborates with the management on a regular basis
Prepares and presents monthly reports
Attends meetings with the management
