Manage and follow-up on the referral of patients to IMC Community Health Workers (CHWs), case management teams for MHPSS and GBV services, and to organizations working in other sectors such as education and shelter as needed.
Work in close coordination with the CHSs and CHWs to strengthen the referral and follow up system of patients at the PHCCs and between the PHCCs and secondary healthcare facilities.
Hold weekly health awareness sessions at PHCCs, or outside of PHCCs to reach patients who have difficult accessing the facility, targeting the Lebanese host community
Work with clinic staff to ensure identification of vulnerable beneficiaries and those in need of additional assistance or case management
Orient the patients to services available to them through IMC, UNHCR, and other stakeholders, and explain financial coverage and eligibility for PHC services and refer as needed
Follow up with pregnant women after delivery to encourage completing 4 ANC visits and their return to the facility for PNC.
Ensure that information on ANC & PNC is entered into the Health Information System (HIS) pregnancy record.
Provide counseling on family planning and provision of modern methods;
Provide counseling and support for exclusive breastfeeding; and BCG and OPV immunization to newborns. Participate in public health assessments; oversee data collection; and compile data
In absence of midwife, Provide counseling on family planning and provision of modern
Coach and support community health workers in utilizing data collection and monitoring tools
Monitor outcome of referral with health facilities and provide feedback on referred cases to community health workers
Monitor reporting for community-based health surveillance system
Provide detailed reports to health area manager on a weekly basis
Communicate information in a timely manner to community health outbreak
Assist in developing materials and visual aids for the program
Minimum Qualifications
Academic and professional
Possession of a BS in Nutrition, pharmacy, Nursing or other relevant health fields of at least three years duration
Experience
A minimum of two years of satisfactory working relevant experience
Previous work with a non-governmental organization or community-health programming
Language
Good command of written and spoken Arabic and English
Computer Skills
Good command of Microsoft Office programs including Excel and Powerpoint
