Health Social Worker

Assignment The health social worker will be based in Kamed el Loz, West Bekaa. He/She will be assigned to the operations of a mobile clinic serving the surrounding villages of the area.    Main responsibilities: Participate actively in the outreach and the organization of the mobile clinic in the allocated village To perform triage and health education sessions (in collaboration with the nurse). To provide psycho-social support and counselling   Main Activities: Conduct field visits to collect data on the number of beneficiaries living in each of the villages mentioned before, their age, their condition of through interaction with municipalities, home visits Share with the local vulnerable population identified the information of the coming mobile clinic, with the services provided, the conditions of access, and the schedule Coordinate with local actors and interact with various authorities Identify families or individuals in need of medical, psychological or social support or counseling through home visits Identify protection concerns (Trauma, SGBV, domestic violence) and provide psychosocial support to cases of concern through the Mobile Clinic. Participate to the loading and unloading of the vehicles and to the setting up of the medical clinics. Participate actively in the triage activities (with the support of the nurse) Manage the flow of the patients properly. Fill in the registration form for each patient, according to the diagnosis of the morbidity surveillance form Spread messages about health education in the villages for the following topics: hygiene, safe water, nutrition, immunization, antenatal care, post natal care, family planning, diarrhoea. Sensitization of the pregnant regarding ANC and the newly delivered to PNC follow up consultations. Give any available accurate information to the patients regarding referrals, distributions, registrations, etc...
