- Provide information and education to patients on pathology, prognosis, complications, treatment and prevention of diseases (in particular Non-Communicable Disease)
- Assess patient’s situation, ressources and needs and set up individualised support plans with each patient according to the latter’s living conditions, needs, and circumstances
- Supporting and counselling patients to achieve treatment adherence and a healthy lifestyle with their diseases, during individual and group sessions in the clinic and in the community of Wadi Khaled
- Following up with the patients and tracking their progress – along with their attendance to the service
- Implement community outreach activities (eg. home visits, support a vaccination campaign, health education and awareness sessions in schools, churches, organize and conduct patient-support-groups in the community) under the indications and supervision of the HP Supervisor
- Collecting all type of information related to specific health and living conditions of the population in Wadi Khaled, in particular to identify the most vulnerable populations to target.
- Ensuring good communication and collaboration with the other components of the project and pro-actively contribute to interdisciplinary team
- Monitoring patient support and HP activities (collecting and documenting data, follow up of indicators, etc…)
- Developing patient support tools (health promotion messages, flyers, flipcharts, games, teaching materials, etc.) with the HP team
- Under the guide of HP Supervisor, assess the impact of activities, and report problems, success and constraints to him/her.
