World Vision (WV) Lebanon is inviting potential suppliers, which are all strong and highly regarded within the industry.
Kindly find attached the related RFP and Supplier Questionnaire along with Safeguarding (WV Supplier Code of Conduct and Child Protection Policy) and SRF (Supplier Registration Form).

You are kindly requested to provide us with the below documents signed and scanned:

• Provide a certified copy of a certificate of business registration, certificate of incorporation, business license or similar document (Commercial circular)
• Provide a certified copy of tax registration, tax clearance certificates or similar documents (Vat Document)
• Provide information on ownership structure (Name of directors of the company / Owner) (Organization chart)
• Provide last two years of audited financial statements or tax filing, or similar documents
• Provide references from previous customers
• Sign WV’s Safeguarding document, Conflict of Interest and Anti-corruption policy (attached) to be signed
• RFP (attached) to be filled and signed
• Supplier Questionnaire (attached) to be filled and signed
• SRF (attached) to be filled and signed
