Responsibility
The hospice and palliative care medical coordinator will be involved in multiple aspects of SANAD’s growing projects and clinical service. In this role, the coordinator will ensure a cohesive, team-based approach in the projects pursued, guiding the medical, nursing, mental health and quality, research and development team and facilitating collaboration and communication between them. The coordinator will be responsible for carrying through the advocacy plan, competency annual evaluation for old staff and evaluation of new staff plan, training needs annual planning and meeting with the medical director weekly for updates on SANAD clinical and non-clinical initiatives.
Functions
Under the supervision of the Medical Director and SANAD's Director and in accordance with the Executive Team Practice Policy” and “Medical protocol”, the physician is expected to carry out the following tasks:
Clinical Tasks
- Approves patient admissions and supervises the medical admission evaluation to determine goals of care, care plan, and monitoring of plan implementation upon need identified by nursing team
- Coordinates care plan with the primary physician of patient and conducts virtual meetings with primary upon need to facilitate patient care
- Performs home visits (at least once a week)
- Provides medical advice and prescriptions for SANAD hospice nurses upon need
- Addresses any clinical concerns by nursing, mental health or medical team members as appropriate and in coordination with the medical director
- Promotes the use of clinical tools adopted by SANAD for clinical team, and addresses any barriers or challenges for their use
- Collaborates with the training focal point to identify alternative clinical tools based on the need
- Promotes the use of the electronic health record tool, and documents patient visits
- Evaluates the effects of the care given and regularly reevaluates the patient’s nursing needs
- Conducts regular and periodic meetings with the team, to ensure implementation of the internal new and old medical and nursing policies
- Ensures a weekly IDT meeting with the nursing team to discuss patients’ update, progress, and challenges faced
- Be available for the families to respond to and provide answers should they need it throughout the care period
- Conducts bimonthly meetings with the Medical Director to ensure regular discussion over progress, challenges faced, alternative solutions and all other technical matters.
Non-clinical Tasks
- Guides the implementation of SANAD’s advocacy plan for hospice palliative care nationally
- Assists in ongoing revisions and development of the competency basedapproach framework at SANAD including main document, policies, procedures and competencies in coordination with the training focal point
- Followsup the monitoring and evaluation plan to ensure proper use of clinical policies and competencies of the medical and nursing team in coordination with the training unit
- Identifies internal training needs based on the evaluation process, and design and implement annual training schedule
- Works with departments to revise workplans per unit and create a timeline annually
- Facilitates interdepartmental communication through:
- Leading the Heads of Units weekly meeting, facilitating and structuring the agenda and communicating follow-up plans
- Being available to support nursing supervisor, medical director, training focal point and QRD head with project development and implementation, communicating updates and weekly plans with the team, and facilitating collaboration on shared tasks or projects
On Quality, Research and Development Unit Level:
- Assists in internal quality assurance and quality improvement projects
- Assists in developing external training material and evaluation of trainings
- Assists in delivering awareness sessions for healthcare professionals
- Meets with potential collaborators and funders for new projects brainstorming
- Assists in planning and write up of proposals for development of palliative hospice care services among identified stakeholders
- Assists in shadowing of physicians involved in newly developed palliative/hospice care programs/services
Qualifications:
- Medical degree from an accredited School of Medicine
- Current license to practice medicine in Lebanon with a minimum of 2 years’ experience
(Experience in hospice and palliative care strongly preferred)
- Able to communicate effectively in Arabic and in English, both verbally and in writing
- Excellent interpersonal skills
- Excellent writing and presentation skills
- Knowledge in computer software applications such as Microsoft Office
