Social Worker – Tyre - Primary Health Center

Assignment

The Social Worker will be based in Amel Association Tyre Primary Health Center. The Social Worker will be assigned to a medical project, which is part of an Emergency Response for Syrian refugees. The Social Worker will be responsible to provide expertise on Social well-being issues to beneficiaries and implementing preventive health activities including awareness sessions, noncommunicable diseases awareness and mental health awareness and screening.

Tasks and duties

• Assess the medical and psychological needs of the Syrian refugees and of patients in the Primary Health Care Center
• Keep up to date mapping of the services provided in the catchment area of the center
• Identify and refer cases of vulnerable persons when necessary
• Follow-up with patients to ensure that their situation has improved
• Provide information on existing social services

• Fill weekly and monthly reports about the cases, referrals, follow-up, etc. of the cases encountered
• Prepare and conduct, in coordination with the Nurse and the Head of Center, Awareness Sessions on health-related issues
• Participate in trainings and internal meetings
• Coordinate with local actors and interact with various authorities

• Work as a part of a multidisciplinary team with the medical and paramedical staff in the center
• Provide individual educational messages to patients on health related topics

• Promote human rights and Amel services to refugees and vulnerable Lebanese families in coordination with the centers’ staff
