The incumbent will be mainly responsible of providing structured social counseling and follow-up for the beneficiaries attending the center.
Tasks:
- Monitors services delivered to the client to ensure that individual needs are met.
- Interviews beneficiaries and prepares statements: summarizing the patient’s perception of the problem.
- Collects information from the family’s beneficiaries, community agencies, and others to be used by treatment team staff.
- Coordinates with staff in the development, implementation, and evaluation of the target groups individual plan service.
- Meets with representatives of community agencies and others NGOs to assist in the development of community support services.
- Provides the patient’s family with information regarding his progress and treatment obtains.
- Attends and participates actively in regular meetings seminars and trainings.
- Presents as written auto-evaluation each 3 months.
- Follows Restart center policies and bylaws.
- Case management.
- Assesses beneficiaries’ psychosocial needs.
- Communicates regularly with beneficiaries regarding case management.
- Prepare with the collaboration of the senior social worker all tools concerning social files.
- Every referral to other specialists (internal and external) and contact with other NGOs should be with the coordination and the supervision of the senior social worker.
- Undertaking and writing up assessments (often with medical staff), which meet specified standards and timescales;
- Conducting interviews with beneficiaries and their families to assess and review their situation;
- Offering information and counseling support to beneficiaries and their families;
- Liaising with, and making referrals to, other agencies
- Participating in multidisciplinary teams and meetings, for example child protection, mental health.
- Participating in team meetings, peer supervisions and case studies.
- Undergoing individual supervision when needed
- Taking over the hotline
- Maintains records and prepares reports and correspondence related to work as assigned under the supervision of the project director.
- Doing school visits in order to coordinate with the teachers and observe the child’s behaviors.
- Conducting home visits.
- Preparing and organizing the trainings conducted by Restart•
- Data entry.
