Main responsibilities:
- Build the capacities of primary health care centers (PHCC) staff and facilitate the integration of MH in primary health care (PHC).
- Provide awareness sessions on MHPSS topics to the front- liners.
- Provide case management to persons with disabilities’ social needs and ensure they access to suitable services through internal or external referrals
- Promote inclusion mainstreaming in PHCCs and support capacity building efforts from MdM and its partners
- Conduct awareness sessions to patients of the PHCCs shadowing the PHCC staff.
- Design and facilitate support groups on MHPSS topics to people with psychosocial issues
- Networking and mainstreaming with available services in the community.

Main responsibilities and activities of the Social Worker can change according to the organization of the work and to the composition of the team.
Main activities:
- Build the capacities of primary health care centers (PHCC) staff and facilitate the integration of MH in primary health care (PHC) along with inclusion mainstreaming.
- Contribute in the needed assessments to be done
- Conduct trainings, sensitization and awareness sessions for the PHCC multidisplinary team on several MHPSS topics, case management and awareness raising facilitation
- Provide supervision / shadow the persons trained on the topics mentioned above.
- Support the PHCC staff in the identification of MH cases
- Organize and facilitate awareness sessions at the PHCC level for the persons in the waiting areas, in cooperation with PHCC staff.
- Provide proper case-management (including safe identification, needs assessments, action plan’s design and PSS support), regular follow-up and internal and external referrals for the persons with disabilities’ social needs enrolled in the project.
- Participate in updating the service mapping.
- Support the PHCC staff to safe identify and refer persons when needed, utilizing the referral form
- Support the PHCCs coordinating with hospitals and any other relevant stakeholders
- When needed, participate to case discussions with the multidisciplinary team
- Coordinate with the staff to facilitate mainstreaming of MH and inclusion at PHCC level
- Community based activities
- Design, organize and facilitate support groups for the community.
- When requested, participate to awareness sessions in ITS/shelters/streets to support outreach workers.
- Participate to relevant sessions on Inclusion organized within Community Support Groups
- Advocate on all MH related issues and/or concerns through events organized by MDM and the other partners.
- System strengthening
- Prepare awareness sessions interactive tools related to different MHPSS and inclusion topics
- Prepare, design and adapt the trainings materials.
- Develop innovative, relevant, adapted and appropriate awareness material with the Community Health team and support them in delivering related messaging;
- Reporting and meetings
- Report all the activities and data collection, respecting deadlines
- Keep records, statistics and files updated ensuring confidentiality of the data collected
- Participate in internal meetings and individual meetings.
- When required participate in external meetings with key actors, NGO’s/INGO’s.
- When required, participate in network/coordination and case discussion meetings concerning MHPSS and/or inclusion
