Context of intervention:
Amid the current economic crisis, more than half of the population of Lebanon is living under the poverty line including a growing number of elderly. These persons used to resort to private health sector, which has become unfordable, thus leading to health care disruption particularly for NCD patients. MdM in partnership with a local NGO has develop a project aiming at identifying, assessing and referring vulnerable NCD patients to PHCCs along with financial coverage of needed medication to ensure health needs maintained and to strengthen their access to healthcare.
Main responsibilities:
The nurse will have the following main responsibilities:
- In collaboration with a social worker, assessing healthcare status of vulnerable households during home-visits
- Identify and refer persons with health needs
- In collaboration with a social worker, internally refer the patients to the medical advisor for financial coverage of the medications
- Provide health education to patients
- Upon need ensure nursing follow-up of patients
- Coordinate with the partner’s social team
- Upon need provide technical support to the partner’s social team
Main responsibilities and activities of the Nurse can change according to the context, the organization of the work and to the composition of the team.
Main activities:
- Assessing healthcare status of vulnerable households
- In collaboration with the partner’s social worker, participate to home-visits
- Conduct individual nursing interview
- Assess health needs and risks through nursing examination
- Document the assessment
- Identifying and referring patients with health needs
- Upon needs, assist in the service mapping and to the development of an effective referral system toward health actors
- In collaboration with the partner’s social worker, properly and timely identify and refer persons in needs of medical services
- In collaboration with the partner’s social worker, internally refer the patients to the medical advisor for financial coverage of the medications
- Provide health education and nursing follow-up
- Provide health education based on the socio-medical assessment
- Upon needs based on the assessment and case discussion with the medical advisor, provide individual follow-up
- The nurse will work in coordination with the social team and will be part of the weekly technical meetings / case discussions with the partner’s social worker and the medical advisor
- Provide technical support to partner’s social team
- Upon need, provide technical support to social workers
- Contribute to development of joint technical documents
- Reporting, meetings and documentation
- Daily documentation of assessments, follow-up and referrals
- Monthly activity reporting
- Participate in and may take the minutes of internal meetings and individual meetings.
- Upon needs, contribute in network/coordination meetings concerning health, while maintaining accurate coordination with the program manager and the partner

Requested Competence/Skills/Profile
- Registered nurse;
- 1- 2 years’ experience in NGO setting;
- Experience in primary-healthcare and NCD screening and management program
- Experience in outreach activities and health education
- Experience in medico-social project is a plus
- Experience working in emergency context is valuable
- Excellence in organizing, adapting, planning and prioritizing tasks.
- Fluency in Arabic, English is a plus;
