Dear Sir,

 

You are invited to submit a quotation for supplying Insurance Coverage as per the following:

First :

Second :

  • Total Number of Kids 5025 Adolescent (Distributed over 19 schools)
  • Each Kid will visit the school and do sports activities for 8 times/month.
  • Locations: Beirut, Mount Lebanon, Bekaa, South, Tripoli & Akkar
  • Duration: 9Months
  • Item components: Medical Coverage during the activities in the session (duration 2 hrs. per session per school)

Third :

  • Total Number of 24 Peers (Distributed equally over 3 cycles ,3 month each cycle)
  • Each Peer will visit the venue and do sports activities for 4 times/month.
  • Locations: Beirut, Mount Lebanon, west Bekaa, Nabatiyeh, Tripoli & Akkar
  • Duration: 9Months
  • Item components: Medical Coverage during the activities in the session (duration 4 hrs. per session per region)

 

So as a conclusion we have 5,769 Adolescent to be insured during 9 Month period distributed over Lebanese governates.

How to Apply

Noting That:

  1. Price should be in USD currency
  2. Whether your quote are inclusive or exclusive of VAT, if not otherwise stated, we will assume your quotes are inclusive of VAT.
  3. Payment terms.
  4. Quotation must be signed by an official authorized to bind the offer.
  5. Quotation expiry date.
  6. Quotation should be submitted in a sealed envelope or Scanned by email to the attention of the undersigned within the period indicated above.
  7. Quotation should clearly mention the RFQ number

The offer and this document shall be signed and presented to the HOOPS Office,

not after 28 April 2022 at the following address:
