Job Purpose
The Health Case Worker will be in direct contact with patients with Chronic Kidney Disease undergoing hemodialysis treatment as well as with the contracted hospital’s focal points. The Health Case Worker will assess the vulnerability of patients and constantly ensure they are receiving the needed hemodialysis treatment as scheduled. He/she will also refer them to other healthcare facilities that can provide them with the specific care and help needed
Responsibilities
- Participate in conducting introductory visits to the hospitals/centers providing hemodialysis treatment before the initiation of the project.
- Conduct phone-based interviews with the patients on hemodialysis existing on the UNHCR referral list to assess their eligibility, update their information, and request the needed personal and medical documents required for their admission.
- Assist in the patients’ selection and update regularly the list of patients enrolled in the program.
- Ensure that the selected beneficiaries are referred appropriately to the nearest contracted hospitals.
- Assist in developing the online tracking tools and keep them well updated while ensuring the confidentiality of information collected is maintained at all times.
- Update regularly the treatment plan of each case by directly liaising with the contracted hospitals and the beneficiaries.
- Prepare and send commitment letters to each beneficiary and hospital detailing the financial contribution of Mercy-USA.
- File documents in an organized and efficient manner in both paper and electronic formats to facilitate the review of cases.
- Review rigorously the supporting documents sent by the hospitals required for the payment processing.
- Assist the MEAL department in conducting regular hospital or home visits to collect the required information and documents, and monitor the quality of treatment provided.
- Identify and refer cases in need of other assistance such as vaccination, and chronic medications.
- Keep the Project Officer well informed about the project challenges encountered.
- Assist the Project Officer in Narrative reporting or other tasks as assigned.
- Attend coordination meetings with the project stakeholders.
- Represent Mercy-USA in working group meetings.
Minimum Hiring Requirements
- University degree in Nursing science or any health-related field.
- Experience in working within the hemodialysis department is an asset.
- 1 to 2 years of experience in the health sector.
- INGO/NGO experience is preferable.
- Demonstrate a high level of cultural sensitivity, professionalism, politeness, and objectivity when dealing with the beneficiaries under all circumstances.
- Organization skills, including time management and the ability to meet deadlines.
- Good command of the English language.
- Proficiency in Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint.
