Referral Nurse – Wadi Khaled

Main Purpose

Organizing, carrying out and surveillance of external referrals of patients from MSF-supported structures to hospitals, in line with MSF referral policy, including the follow-up of referral cases to ensure that good quality of care is provided to the beneficiaries.

Maintaining professional relationships with the network of hospitals for referrals, under the supervision of project medical referent and project coordinator, and in coordination with the social worker.

 

Accountabilities

  • Organizing and carrying out care and treatments according to medical prescriptions, and assisting during consultations, daily rounds, and other medical procedures. Participating in health education of the patient (and family) when necessary.
  • Identifying Sexually Gender Based Violence victims and referring them to the medical team so they can receive the necessary treatment. Knowing and is aware of the importance and appropriate use of the Post Exposure Prophylaxis (PEP) Kit
  • Ensuring patients are properly received and installed and that those with lack of autonomy are assisted, especially regarding their feeding, personal hygiene, movements, and comfort.
  • Carrying out, surveillance and follow-up of patients (i.e. assessment of the evolution of their state of health and the identification of any emergency or deterioration
  • Carrying and supervising administrative procedures and documents (fill in patient’s files, forms, consumptions, statistics, registers, health files, etc.), ensuring an appropriate written/oral handover, and reporting any problematic situations and cases that may arise.
  • Participating in data collection and keeping doctors/supervisors informed.
  • Participating in the department-related pharmacy and medical equipment control and maintenance (quality and its functioning, storage conditions, follow-up of expired drugs inventories, stock takes of medicines and material at every team changeover ensuring no material is taken out of nursing area/wards without prior authorization, etc.)
  • Respect confidentiality at all times.

 

MSF Section/Context Specific Accountabilities

General

  • Taking responsibility for the management of referrals, including anticipating questions or issues that may arise, and consulting regularly with the relevant technical referent (Project Medical Referent - PMR, Midwife Activity Manger - MWAM, Nursing Activity Manager - NAM, etc.), as needed.
  • Participating in the data collection and maintaining the referral registers.
  • Maintaining accurate and complete documentation of medical records as per MSF procedures in a timely manner to allow regular tracking of the referred case.
  • Ensuring patients are prepared and have received all information prior to transfer, complete patient’s files, and all necessary information is shared with the MSF team and the receiving facility.
  • Monitoring patient throughout the referral transport to be aware of any changes in health status and any potential medical intervention needed.
  • Ensuring patients are properly admitted in ER, IPD/ OPD or at any referral hospital, without delay.
  • Communicating daily with external hospitals and other receiving facilities to monitor the medical treatment and to provide updates on patient care and evolution to the relevant MSF colleague and Makassed staff.
  • Weekly visits to Notre Dame de la Paix, Halba Governmental Hospital, and other NGO partners (as needed) to maintain the proper lines of communication and improve collaboration. The visit can be accompanied by the PMR, MWAM or Midwife Supervisor.
  • Communicating the treatment plan to the patients and family in a clear, accurate, and compassionate manner.
  • Being able to respond to conflictual or stressful situations in a positive and constructive manner.
  • Assisting in the verification of patient files and hospital/patient receipts and billing on a monthly basis, in collaboration with the social worker, PMR, MWAM, NAM or FinHR Manager to ensure these correspond to the referral register, previously approved amounts, and treatment provided.
  • Maintaining good communication and collaboration with supervisors, PMR, social worker and other team members in the facility to promote quality of care and ensure patient is receieving necessary care and support.
  • Be contactable by phone and available to assist with important referral-related issues that may occur outside of regular working hours (e.g., emergencies during nights, weekends, or holidays).
  • Respecting and following at all time the universal hygiene standards/ precautions, bio-hazard prevention and infection control, security rules and procedures in the medical premises.

The above-mentioned activities and responsibilities are not exhaustive, and the employee can be asked to perform other tasks, according to the necessary daily work and the occasional needs of the mission.

-Requirements:

Experience:          Minimum 2 years of previous experience as Registered Nurse.

Languages:           English and Arabic Language.

Qualification:      Essential recognized Nurse degree/diploma

                             Registration with Ministry of Public Health – MOPH with a Practice license as Nurse in Lebanon

Knowledge:          Advanced Computer Literacy (word, excel and internet)
