Case Managers - 2 positions (Offer Pending the Donor Final Approval on the Project Proposal)

Purpose: In compliance with best-practice, national and international standards, provide case management to vulnerable persons through high quality and confidential interventions with the aim of addressing protection risks, empowering the beneficiaries, allowing individuals to access their rights, regain control over their lives and restore dignity.

 

Responsibilities:

  • Identify individual cases facing a protection concern and need intervention;
  • Provide individual counseling for identified cases to assess the needs, risk level, and determine eligibility for case management, and provide information about available services;
  • Conduct home visits to vulnerable individuals, assess their needs and engage with their family members and caretakers; develop, implement and follow up on action plans and monitor situation prior to case closure;
  • With their informed consent, refer the beneficiaries to appropriate available services (mental health, medical supplies etc.) and facilitate their inclusion in their community;
  • Provide awareness sessions to beneficiaries, family members and caretakers whenever needed;
  • Provide psycho-social support and peer-to-peer support to beneficiaries through group interventions to be conducted in accessible safe spaces identified in collaboration with group participants;
  • Apply age and gender-appropriate facilitation skills during group sessions; use interactive adult learning techniques to effectively engage with session participants;
  • At all times, ensure the safety and respect of beneficiaries and their effective inclusion in all activities, in addition to the confidentiality of their information and their right to access services without any discrimination;
  • File, analyze and store the beneficiaries’data as directed by the project officer; ensure the utmost confidentiality of case management data and information throughout the case management process, including protection referrals;
  • Provide necessary counseling and support for all beneficiaries referred to the case management service;
  • Prepare and submit weekly, monthly and quarterly work plans in a timely manner with validation from the Project Officer;
  • Support the implementation of monitoring and evaluation tools, provide remarks, recommendations and success stories and report on challenges during the implementation of the project;
  • Develop and maintain effective working relationship with relevant stakeholders including partner agencies, community leaders and service providers;
  • Actively contribute to the maintenance of an up-to-date service mapping of the service providers operating in the area of implementation (Tripoli, Dennieh and Akkar);
  • Assess referred cases and take immediate action to address urgent situations;
  • Participate in regular supervisory meetings to ensure adherence to standards of care and appropriate case management;
  • In collaboration with the Project officer, attend case conferences with relevant key actors when needed;

 

The responsibilities and tasks set out above are not comprehensive and the case manager may be required to perform additional tasks as needed according to the structure of the project and what is asked by the project officer or project manager.
