Social Worker - Beirut

Job Description:

We are looking for a dedicated and skilled professional to join our team in providing comprehensive support and personalized care to vulnerable individuals through effective case management.

Key Responsibilities:

Rapid Assessments: Conduct rapid assessment interviews with targeted populations to identify and register vulnerable persons (Social file).

Case Planning: Develop individualized case plans for each beneficiary with clear outcomes and timeframes.

Monitoring and Reassessment: Regularly follow up, monitor, and review cases, reassessing vulnerabilities and updating case plans as needed.

Support and Assistance: Offer information, support, and assistance to those in need.

Referrals: Liaise with and make referrals to other organizations; follow up with beneficiaries to monitor progress.

Coordination with Healthcare Professionals: Coordinate with doctors, therapists, and health professionals regarding patient appointments and progress.

Family Engagement: Raise awareness among beneficiaries' families during treatment.

Feedback and Reporting: Provide feedback on referrals and active patients to referring partners and coordinators. Submit monthly reports to respective coordinators.

Training and Team Collaboration: Participate in training modules, case conferences, supervision, and team meetings.

Data Management: Manage, fill, and archive specific forms for case profiling in the data software system (e.g., social files, attendance sheets, pre and post-tests, session records), ensuring confidentiality, accuracy, and timeliness.

Home Visits: Conduct home visits based on the needs of beneficiaries.

Mental Health Assessments: Prepare mental health assessment reports detailing the status of treated patients.

Admissions and Discharges: Plan admissions and discharges from psychiatric and health facilities.

Social Counseling: Conduct social counseling with beneficiaries.

Resilience Assessments: Administer resilience/impact forms to beneficiaries before therapy initiation and after three months of completion.

Project Implementation: Actively participate in weekly meetings with coordinators and team members to discuss challenges, achievements, difficulties, and lessons learned.
