Case Manager

Overview:

The Case Manager will be responsible for coordinating and providing comprehensive support and services to beneficiaries in need. This role involves assessing beneficiaries' needs, developing and implementing individualized care plans, and connecting beneficiaries with appropriate resources and services. The Case Manager will work closely with beneficiaries, their families, and other service providers to ensure holistic and effective care.

 

Job Description:

  • Conduct comprehensive assessments of beneficiaries' needs, strengths, and challenges.
  • Develop individualized care plans in collaboration with clients, outlining goals and strategies for achieving them.
  • Coordinate and facilitate access to services and resources, such as mental health support services, protection, shelter, and other types of required services.
  • Monitor and evaluate beneficiaries' progress, adjusting care plans as necessary to ensure effective support.
  • Advocate on behalf of beneficiaries to ensure they receive appropriate and timely services.
  • Provide emotional and practical support to beneficiaries, helping them navigate complex systems and processes.
  • Maintain accurate and confidential records of assessments and care plans.
  • Collaborate with other service providers, agencies, and stakeholders to enhance service delivery and support beneficiaries' needs.
  • Prepare regular reports on beneficiary progress, program activities, and outcomes.

Qualifications:

  • Bachelor's or Master’s degree in Social Work, Psychology, Counseling, or a related field.
  • Minimum of 3-5 years of experience in case management, social work, or a related field.
  • Experience in working with vulnerable populations and coordinating care services is preferred.
  • Strong understanding of case management principles and best practices.
  • Excellent communication and interpersonal skills.
  • Ability to develop and implement individualized care plans.
  • Knowledge of available community resources and services.
  • Strong organizational and problem-solving skills.
  • Empathy, compassion, and a commitment to social justice.
  • Ability to work collaboratively with beneficiaries, families, and other service providers.
