Community-Based Rehabilitation Specialist

Key Responsibilities:

1. Beneficiaries’ assessment and case planning:

  • Conduct thorough beneficiaries’ assessments to understand personal, social, and financial needs.
  • Develop individualized case management plans that address specific challenges and goals.
  • Ensure that case plans align with both short- and long-term goals for each beneficiary.

 

2. Monitoring and follow-up:

  • Regularly monitor beneficiaries’ progress, reassess needs, and adjust case plans as required.
  • Maintain accurate and detailed records of each case, including assessment findings, service plans, and outcomes.
  • Conduct follow-up visits or calls to ensure that services are meeting Beneficiaries’ evolving needs.

 

3. Service Coordination:

  • Coordinate services with local agencies, healthcare providers, educational institutions, and government programs to provide holistic support to beneficiaries.
  • Connect beneficiaries to resources, including financial aid, housing, employment, health care, and counseling.
  • Collaborate with multidisciplinary teams to advocate for beneficiaries and secure needed services.

 

4. Advocacy and support:

  • Serve as an advocate for beneficiaries, ensuring that their rights are respected and that they receive fair treatment and access to services.
  • Provide emotional and social support to beneficiaries, fostering resilience and empowerment.
  • Educate beneficiaries and their families on navigating systems and understanding their rights.

 

5. Crisis Intervention:

  • Respond to crises by assessing risks, providing immediate support, and connecting beneficiaries with emergency services.
  • Develop safety plans for beneficiaries at risk and provide critical support during high-stress situations.

 

6. Documentation and reporting:

  • Keep detailed case notes, assessments, and plans while ensuring confidentiality and legal compliance.
  • Write reports on case outcomes, challenges, and suggestions for future actions.
  • Track trends and issues to help improve projects and allocate resources effectively.

 

Requirements:

 

Education:

  • Bachelor's Degree in Medical Social Work or Social Work.

 

Professional Experience: 

  • One to three years of experience, with previous exposure to Lebanese communities preferred.
  • Previous experience in case management, social services, or a similar role, ideally working with diverse populations.

Language Proficiency:

  • Arabic: Fluent
  • English: Fluent
  • French: Strong Asset.

 

Working Schedule: 3 days per week; from 08:00 AM till 04:00 PM

Contract Type: Fixed-Term Contract, 10 Months (Renewable based on funding and the need for position).

Location: Jisr El Wati, Sin El Fil, Beirut
