The coordinator will ensure the effective implementation, coordination, and follow-up of support for displaced individuals, focusing on the following areas:
- Coordination
- Collaborate closely with PSU frontline workers and other partners to track cases discharged from the hospital, including NICU and PICU cases, maternity services, and acute pediatric cases to provide post care discharge.
- Conduct regular meetings with partner organizations to discuss support areas, minimize duplication, and ensure inclusive coverage of all IDPs.
- Data Management
- Document and maintain records of identified IDPs, including their specific needs and referral pathways.
- Map out and maintain a database of partners providing various social services to ensure comprehensive support for IDPs.
- Collect detailed information on the needs of IDPs to enable targeted referrals and assistance.
- Monitoring Support
- Conduct phone calls with IDPs to gather data on the conditions and needs of displaced families, identifying any additional support requirements.
- Document and develop human interest stories that highlight the experiences and needs of IDPs for reporting and advocacy purposes.
- Establish and maintain a comprehensive database documenting identified IDPs, their acute needs, and referral history.
- Establish and regularly update a monitoring dashboard to track follow-up actions, cases closed, and outstanding needs, reviewed monthly to inform reporting and decision-making.
- Develop a detailed report mapping partners by region and services provided, including contact details, to facilitate smooth referrals.
- Provide a monthly summary report detailing the specific needs identified among IDPs to guide targeted interventions as well as outlining the effectiveness of the support provided to IDPs, ensuring no cases are missed and all receive appropriate referrals.
- Organize and participate in monthly coordination meetings with PSU frontline workers and relevant partners to review follow-up cases and align support efforts.
- Conduct phone calls with beneficiaries to document findings on their living conditions, unmet needs, and follow-up requirements.
- Develop at least three human interest stories, capturing the experiences, challenges, and resilience of displaced families, for reporting and advocacy
