Job Summary:
The Registered Nurse in older people Roving Unit is responsible for providing excellent quality holistic home nursing care to people aged 60 years and older and applying the nursing process (assessment, planning, intervention, and evaluation) under the supervision and direction of the medical doctor and the field coordinator. The job is a field-based job where the Registered Nurse will be asked to do home visits and provide nursing care tailored to patients' preferences and pace while maintaining patient confidentiality and always advocating for all patients. Patient and family education is a key responsibility that must be adhered to. Moreover, the Registered Nurse will submit reports about all activities done, success stories, and challenges faced and will be asked to attend related training and meetings.
Tasks and Responsibilities
- Working Hours, Schedule, and Conditions:
- This is a full-time position that involves both fieldwork and office work.
- The work schedule is Monday through Friday, from 8:30am to 3:00pm, with 5.5 hours designated for field work and 1 hour for data entry and office work
- The position requires collaboration with medical doctors and close coordination with the field coordinator
- The job responsibilities include working in accordance with the project's log-frame to achieve targets set
- The position requires conducting at least 10 home visits daily
- The nurse is responsible for their own transportation to and from work
- A daily plan must be prepared the day before and implemented for each working day.
- The nurse is expected to abide by Amel's values, regulations, laws, and vision when working with hosting and refugee communities
- The nurse is responsible for communicating with the community to ensure their understanding of the project/PHCC services
- Field Tasks and Responsibilities:
- Professional Performance and Safe delivery of Patient Care:
- Providing direct and indirect older age specific nursing care for assigned patients within the framework of patient centered care.
- Applying the nursing process and critical thinking throughout the care
- Developing care plans that are specific to each homebound older adult and align with the medical doctor's plan of care
- Ensuring the confidentiality of patient information and reporting any breaches.
- Abide with safety and Infection control standards and report any suspected outbreaks.
- Professional Performance and Safe delivery of Patient Care:
- Putting the Nursing Process to Action: (Assessment of Eligibility and Care Planning for Older Individuals)
- Assess all referred older individuals to determine if they meet the inclusion criteria for medical follow-up.
- Assess older individuals' health and living environment
- Assess older individuals' sense of orientation, concentration, and needs for education.
- Identify signs of abuse and any other medical or mental problems or weaknesses
- Develop care plans in collaboration with medical doctors, tailored to older individuals' physical and medical health needs.
- Implement the care plan and evaluate its effectiveness.
- Refer older individuals who need case management or display signs and symptoms of mental health problems to the social worker and clinical psychotherapist, as needed.
- Putting the Nursing Process to Action: (Assessment of Eligibility and Care Planning for Older Individuals)
- Nursing Duties and Responsibilities:
- Taking vital signs (BP, HR, RR, Temperature, SPO2, and HGT) and document them
- Perform a full history assessment, including medical and surgical history, allergies, medications, etc.
- Organize medications according to the patient's schedule using a pill organizer.
- Distribute assistive devices to older people according to their needs and disabilities
- Perform catheter insertions within the scope of nursing practice.
- Provide wound and pressure injury care.
- Educate older individuals and their families/caregivers.
- Assist with basic daily activities, as needed, to improve quality of life (bathing, feeding, walking, etc.
- Administer medications in accordance with medical doctors' orders.
- Minimize hazards and risks for falls.
- Complete medical files, both physical and electronic as instructed, with a nursing note.
- Log all visits in the designated Kobo toolbox form.
- Clean, and sterilize equipment at the start of the day, at the end of the day, and between patients.
- Report to the field coordinator any communicable disease among the community members including but not limited to Hepatitis A, Lice, Covid-19, diarrhea, and scabies.

- Maintain accurate and up-to-date records of all visits through logging them on the Kobo platform by the end of each day
- Create and maintain a detailed cohort list that includes the status and medical information of all enrolled patients
- Monitor and update the distribution lists and inventory of assistive devices
- Keep track of medications dispensed and received through maintaining the medication inventory sheet
- Monitor and update the inventory sheet for medical supplies received and used
- Update the project dashboard on a monthly basis to ensure the most current information is available
- Follow up on the patients referred through the database and provide feedback (as necessary)
- Document all diagnostics planned and conducted, and share results with patients in a timely manner

- Prepare daily visit plans and files in advance to ensure all necessary information and materials are readily available.
- Prepare and ensure all necessary equipment and medical supplies are ready for use prior to each visit.
- Submit regular reports to line managers to keep them informed of progress and any issues that may arise.

Qualifications and personal skills:
- Experience:
- 1-2 years of nursing experience in direct patient care
- Previous experience in bed-side care is an asset
- Previous experience with older people population is considered an asset
- Bachelor's Degree in Science of Nursing (BSN)
