General Overview:

The Case Worker aims to maximize the development of human potential and the fulfillment of human needs, through an equal commitment to Working with and enabling people to achieve the best possible levels of personal and social well-being and working to achieve social justice through social development and social change. In this project, the Case Worker will work in close coordination with the field coordinator, psychotherapist, outreach team, and municipalities in the case management of referred and enrolled older people. Staff is expected to establish a good rapport with older people, have good communication skills, and do service mapping in addition to providing psychosocial support sessions and activities. Moreover, the Case Worker will submit reports about all activities done; success stories and challenges faced, and will be asked to attend related trainings and meetings.

Tasks and Responsibilities:

  1. Abide by target assigned in project’s log frame with at least 30 hours per week, Monday to Friday from 9:00 am to 3:00 pm

1.1. If fieldwork (the plan of the day) was done before working hours were complete, working in the office on transferring collected data from Kobo is to be completed.

1.2. Works 4 day's on-site (fieldwork) and 1 day as office work per week.

1.3. Responsible for one’s own transportation from and to the site (field or office)

1.4. A daily plan should be prepared a day prior and ready to put into action for every working day

  1. Case Worker Field tasks and home visits

2.1. Conduct at least 5 home visits per day, at least 28 home visits per week.

2.2. Check for eligibility of older patient to further follow-up and benefit from Case management interventions based on Katz independency score and WHO-5-Wellbeing-Index score

2.3. Perform a full assessment for older people’s social needs including psychosocial stressors in-line with the bio-psycho-social model.

2.4. Address psychosocial stressors (i.e., unemployment, family conflict, relationship problems, etc.).

2.5. Provide psycho-education sessions on priority MNS conditions (Depression, Anxiety, PTSD, Psychosis, Developmental Disorders, Substance use).

2.6. Provide low-intensity psychological interventions and psychosocial support to persons with mental health conditions and their caregivers

2.7. Formulate and implement a case management plan after liaising with the GP, psychiatrist, and clinical psychologist.

2.8. Informs the supervisors about any difficulty met in activities implementation.

2.9. Assess for risk of suicide, manage suicide risk, and conduct safety planning in adherence to the high-risk protocol and refer to the clinical psychotherapist

2.10. Ensure referrals to and from community services and PHCC and follow-up (health, protection, education, social, income generation, etc.) as needed.

2.11. Ensure adequate follow-up of patients and referrals.

2.12. Participate and stay up to date with community mapping exercises for health, social, educational, protection, and other community-based services in the catchment area.

2.13. Identify older people who are in need for hot meals services.

2.14. Actively participate in MHPSS coordination and technical meetings.

2.15. Attend capacity building training workshops.

2.16. Plan and conduct home-based psychosocial support sessions for older people

2.17. Flexible to perform other duties assigned by line manager related to same field/domain

  1. Documentation, Data, Targets, and Reporting

3.1. Fill in Kobo Case worker form after each visit and/or phone call

3.2. Fill in Case Worker list, and patient files in line with the project’s requirements

3.3. Work in accordance to project is log frame and targets

3.4. Attend bi-weekly meetings and report on statuses, number, challenges, and success stories

3.5. Report with feedback on people being referred to services outside our project

3.6. Responsible for daily reporting to the field and project coordinator with daily tasks

3.8. Writing a narrative report elaborating on numbers, tasks, and activities done mid and throughout the year

Knowledge, Skills, and Abilities:

  1. Knowledge:

1.1. Bachelor’s degree in Medical Social Work, masters is a plus

1.2. A minimum of 2 years' experience as a medical social worker in a PHC or related fields

1.3. Fluency in Arabic, and excellent in spoken and written English is a must. French is a plus.

  1. Skills:

2.1. Basic computer skills and familiarity with Google applications, kobo humanitarian, and Microsoft office

2.2. Excellent interpersonal communication skills (building rapport, listening skills, and able to communicate information in a clear way) and facilitation skills.

2.3. Have good organizational and time management skills, accountability, reliability, and punctuality.

2.4. Knowledge about the predictable stressors of humanitarian aid workers and the policies and practices needed to mitigate them.

  1. Abilities:

3.1. Ability to maintain social and self-integrity and a sense of responsibility at all times

3.2. Ability to maintain the confidentiality of older people enrolled in the project

3.3. Ability to adhere to ethical considerations and principles regarding work with vulnerable populations.

3.4. Ability to demonstrate cultural sensitivity, non-discrimination, and readiness to work with men, and women from diverse backgrounds, and particularly the most marginalized segments of the community.

3.5. Ability to walk and climb stairs
