Major Responsibilities:
Individual Case Management
Conduct household visits and center-based interviews to assess the needs of individuals and families.
Work with identified individuals and families to develop and implement an action plan in accordance with their needs, capacities, and goals.
Discuss with individuals the choices and options available, including seeking out alternatives when needed services are unavailable, and actively involve individuals and/or families in decision-making processes.
Assess risk and support individuals and families to understand the risks relevant to their situation.
Link individuals and families with support groups, networks, and other community-based resources, as appropriate.
Identify and receive referrals regarding individuals and families (including caregivers) with specific needs who are detected during registration, psychosocial support, outreach, and other activities (e.g., through Refugee Outreach Volunteers).
Provide information to individuals and families about their rights and entitlements, including available services.
Ensure appropriate monitoring, review, and follow-up of the services provided to each case.
Ensure complete and updated documentation related to each individual case.
Determine if and when it is appropriate to discontinue follow-up (case closure).
Be accountable to individuals, families, and communities for the services provided.
Provide direct psychosocial counseling and support the coordination of services by specialized providers based on the needs of the individual or family.
Data Management and Reporting
Manage, file, and store data, ensuring the confidentiality of information and services provided.
Prepare and submit weekly and monthly work plans in a timely manner for validation by the Case Management Officer.
Support the implementation of monitoring and evaluation tools and report challenges in program implementation.
Participate in weekly reporting and planning meetings to ensure quality service provision and reporting standards.
Coordination and Representation
Contribute to maintaining an up-to-date service mapping of providers in the area and stay informed about available services.
Receive referrals from other agencies (after allocation by the Case Management Officer) and provide appropriate feedback while respecting confidentiality.
Advocate on behalf of individuals and families to access services and empower them to effectively communicate their needs and perspectives in all relevant settings.
