How to Apply

The Supplier shall complete and submit the following documents with his quotation:

  1. Request for Quotation Form - to be signed and stamped by the Bidder
  2. Annex #1 Technical Data Form - to be signed and stamped by the Bidder
  3. Annex #2 Financial offer (Price to be inserted by the bidder) - to be signed and stamped by the Bidder
  4. Annex #3 SUP 12 General terms and conditions for supply contracts - to be signed and stamped by the Bidder
  5. Annex #4 Code of Conduct for Contractors - to be signed and stamped by the Bidder
  6. Annex #5 Vendor Bank Verification Form - to be signed and stamped by the Bidder
  7. Copy of a valid commercial registration 2025.
  8. Copy of the ID of the company owner.
  9. Letter of confirmation from the Bank dated 2025.
  10. Physical Samples for all the requested items, to be exact to the requested specifications.  

 

Interested Bidders can also request Annex #5 Vendor Bank Verification Form and the financial offer template in excel by email.
