The purpose of this Request for Quotation (RFQ) is to invite qualified suppliers to submit quotations for the supply of Puzzle Mats for the Takallam Center. Takallam is located at Michel Daher Social Foundation, Ferzol, Bekaa, Lebanon. The puzzle mat will be used in all the therapy rooms in Takallam Center.
Scope of Work
The selected supplier will be responsible for:
● Provision of 80 Puzzle Mats:
Supply the specified type of the Puzzle Mat as outlined below, ensuring they meet required quality standards.
Provide detailed product specifications, including brand, model, and warranty details.
● Delivery and Logistics:
Manage timely delivery of the Puzzle Mat to the designated location.
Ensure that the Puzzle Mat is delivered in proper condition, free from damage.
● Assembly/Setup (if applicable):
Assist with the setup or assembly of the Puzzle Mat, if required.
Ensure the Puzzle Mat is fully functional and ready for use upon delivery.
● Quality Assurance and Inspection:
Provide quality assurance certificates or similar documentation for the delivered Puzzle Mat to confirm it meets the outlined specifications.
● Support and Warranty:
Offer post-delivery support for any issues related to the Puzzle Mat.
Provide warranties or service agreements for defects or maintenance needs for a specified period.

Submission Requirements
Technical Proposal:

Delivery Terms
● Delivery Location: Michel Daher Social Foundation, Ferzol, Bekaa, Lebanon.
● Delivery Date: The Puzzle Mat must be delivered by June 10, 2025.
● Packaging: Ensure safe packaging to prevent damage during transportation.
Payment Terms
● Payment will be made via bank transfer or Telegraphic Transfer (TT) within 15 working days after delivery and completion of all deliverables.
● Payments will be processed only after all agreed-upon deliverables are met.
● Tax Note: If no valid Ministry of Finance (MOF) certificate is provided, a 3.4% tax on goods and 8.5% tax on services will apply as per Lebanese tax law.

Submit your quotation via email to procurement@daherfoundation.org by June 8, 2025. The subject line must read: “RFQ for Puzzle Mat – N° MDSF25-RFQ-011”. Incomplete or incorrectly labeled submissions will not be considered.
