1.
TASKS TO BE EXECUTED AND TERMS OF DELIVERY
1.1
The subject of the request is the Medical and Dental Materials provided as detailed in the technical specification under Annex 02.
1.2
Place of the delivery of offers: COOPI Office – Achrafieh, Furn Al Hayek, Baroudi Street, Saab Building, 7th Floor Beirut,
1.3
The awarded bidder will sign a frame work Contract with COOPI.

PARTICIPATION AND SUBMISSION OF OFFER:
Participation in this tendering procedure is open on equal terms to all companies respecting COOPI requirements.
2.1
Deadline for submission of the offer: 10/10/2025 at 16:00, if the offer is delivered after the deadline, shall be invalid and not taken into consideration.
2.2
Bidders are required to submit their technical and financial proposals in the English language.
2.3
Offer, including annexes and all required supporting documents, must be hand delivered in a sealed envelope at the following address before the deadline: COOPI Office – Achrafieh, Furn Al Hayek, Baroudi Street, Saab Building, 7th Floor Beirut
3. CONTENT OF THE OFFER: TECHNICAL AND FINANCIAL OFFER
It is mandatory to provide financial and technical offers signed and stamped.
3.1 Mandatory documents to be submitted

Financial offer should be signed and stamped. The prices shall be expressed in US Dollars.
•
Technical Offer: stamped, indicating all necessary information and documents for disbursement including the brand , country of origin , specification of items

Annex 3: Declaration of Supplier Compliance to Ethical Standards filled signed and stamped.
•
Annex 4: Declaration of non-exclusion filled signed and stamped.
•
Registration certificate “MOF”
•
ID of the owner
•
Bank Account
