1. TASKS TO BE EXECUTED AND TERMS OF DELIVERY

1.1 The subject of this request is the supply of Medical and Dental Materials as detailed in the Technical Specification (Annex 02).

1.2 Place of submission of offers:
COOPI Office – Achrafieh, Furn Al Hayek, Baroudi Street, Saab Building, 7th Floor, Beirut

1.3 The awarded bidder will be required to sign a Framework Contract with COOPI.

2. PARTICIPATION AND SUBMISSION OF OFFERS

Participation in this tender is open to all companies that meet COOPI’s requirements.

2.1 Deadline for submission:

• Offers must be submitted by 27/10/2025 at 16:00.
• Offers delivered after the deadline will be considered invalid and will not be evaluated.

2.2 Language of submission:

• Both Technical and Financial Offers must be submitted in English.
• The Technical Offer may be submitted using the supplier’s own format but must include the specifications and a technical datasheet for each listed item.

2.3 Submission method:

• All offers, including annexes and supporting documents, must be hand-delivered in a sealed envelope to the address above before the deadline.

3. CONTENT OF THE OFFER: TECHNICAL AND FINANCIAL OFFERS

Both Financial and Technical Offers are mandatory and must be signed and stamped.

3.1 Mandatory documents to be submitted:

Financial Offer:

• Must be signed and stamped.
• Prices must be expressed in US Dollars (USD).

Technical Offer:(providedby the supplier)

• The Technical offer must be stamped and include all relevant technical informationfor evaluation and disbursement and should be in order as per the financial offer
• For each listed material/item required by COOPI in the financial offer, the technical offer must clearly provide:
  • Brand name
  • Country of origin
  • Specifications
  • Technical datasheet (mandatory for verification of compliance with requirements)

Other required documents:

• Annex 3: Declaration of Supplier Compliance to Ethical Standards – filled, signed, and stamped
• Annex 4: Declaration of Non-Exclusion – filled, signed, and stamped
• Company Registration Certificate (MOF)
• Copy of the Owner’s ID
• Bank Account Details
• Declaration from the supplier on his own form granting COOPI staff full access to monitor, supervise, and inspect the loading, transportation, and delivery of all goods, including verification of quantities, packaging, and condition at every stage.(this declaration is made by the supplier)
