MMHC Case Worker (Psychiatric Nurse)

A. GENERAL DESCRIPTION

The Case Worker (with Psychiatric Nursing Background )  is a key member of the Mobile Mental Health Clinic (MMHC) team, dedicated to ensuring the continuity and quality of mental health care for beneficiaries across Lebanese governorates. The role combines clinical expertise with coordination and advocacy, focusing on post-consultation follow-up, psychosocial support, and psychoeducation. The Case workere facilitates the smooth integration of beneficiaries into the Primary Health Care Centers (PHCCs) that beneficiaries are referred to, monitors their progress, and collaborates closely with PHCC focal points and the MMHC multidisciplinary team to ensure holistic, patient-centered, and sustainable care.

B.  MAJOR DUTIES AND RESPONSIBILITIES

1. Clinical Follow Up & Psychosocial Support

  • Conduct phone-based clinical assessments or medical interventions with MMHC beneficiaries, when needed, in coordination with the psychiatrist, to ensure appropriate follow-up care
  • Monitor and report beneficiaries’ adherence to medications
  • Conduct follow-up calls with MMHC beneficiaries after their psychiatric consultations at the MMHC to:
    • Assess the patient’s experience and identify barriers to accessing PHCCs
    • Provide psychosocial support and psychoeducation
    • Offer guidance on self-management and available resources
  • Facilitate and coordinate the provision of social support when required, ensuring a comprehensive and patient-centred approach to care

2. Care Coordination & PHCC Integration

  • Support beneficiaries in their integration in PHCCs and coordinate with PHCC focal points to ensure smooth access
  • Identify and reach out to new PHCCs to expand the network and regularly update the service mapping
  • Build and maintain positive relationships with PHCC focal points and other relevant stakeholders across regions
  • Gather feedback from PHCC focal points regarding referred beneficiaries to support ongoing coordination and quality improvement
  • Advocate for beneficiaries to overcome barriers in accessing PHCC services
  • Monitor and report on beneficiaries’ progress and challenges related to PHCC integration

3. Data Management & Reporting

  • Maintain accurate records of follow-up interactions, beneficiary progress, and PHCC access
  • Collaborate with the Project Coordinator and the whole MMHC team to analyse data and develop mitigation strategies to address challenges
  • Provide regular updates and reports to the MMHC team and support donor reporting requirements
  • Ensure compliance with confidentiality and data protection standards

4. Cross-Functional Collaboration

  • Work closely with the MMHC multidisciplinary team—including Project Coordinator, Psychiatrists, and Mobile Clinic Assistant—to ensure holistic patient-centred care
  • Identify complex cases requiring escalation and discuss them during team meetings

5. Operational Support

  • Participate in relevant team meetings and trainings
  • Support communication efforts to promote MMHC services and community awareness when needed

6. MMHC Medications Management

  • Oversee the management of the MMHC medication stock, including proper storage, dispensing, and monitoring of consumption, with the support of the Project Coordinator
  • Assist in identifying medication needs and preparing procurement requests as required

C. REQUIRED QUALIFICATIONS

  • Bachelor’s degree in Nursing, with a specialization or demonstrated experience in psychiatry or mental health care
  • Minimum of 2 years of professional experience in mental health or community-based healthcare settings
  • Previous experience in NGOs or PHCCs is a plus
  • Proven ability to provide psychosocial support, psychoeducation, and patient follow-up
  • Strong communication, interpersonal, and networking skills, with the ability to build positive relationships with PHCC focal points and partners
  • Experience in care coordination and multi-stakeholder collaboration is highly desirable
  • Good knowledge of mental health resources and community referral pathways
  • Familiarity with Lebanon’s Primary Health Care (PHC) system is an asset
  • Strong skills in data collection, reporting, and documentation
  • Fluency in Arabic and English (spoken and written)
  • Proficiency in Microsoft Office tools (Word, Excel, Outlook)
