Association des Jeunes Islamiques (ADJI) is a leading non-profit rehabilitation center in Lebanon. Established in 1997, the organization is dedicated to providing high-quality, patient-centered rehabilitation service including physical therapy, occupational therapy, speech therapy, and psychosocial support to vulnerable individuals such as persons with disabilities (PwDs), children with disabilities (CwDs), and the elderly. In addition to its rehabilitation programs, ADJI operates Rahma Primary Health Care Center (PHCC) an integrated facility within the national primary health care network and licensed by the Ministry of Public Health offering comprehensive and inclusive healthcare services to underserved communities in North Lebanon.

As part of its commitment to integrity and ethical conduct, ADJI enforces a zero-tolerance policy on Sexual Exploitation and Abuse (SEA) and requires all staff to adhere to the Protection from Sexual Exploitation and Abuse (PSEA) Code of Conduct.

Project Context

This position is part of a 12-month integrated health and nutrition project funded by the Lebanon Humanitarian Fund (LHF/OCHA – CBPF) under Project Code: CBPF-LEB-25-S-NGO-37920, implemented in partnership with Alaaransa/ Primary Health Care Center (PHCC) in Akkar and Wadi Khaled.

The project targets 10,800 vulnerable individuals and delivers 20,000 subsidized consultations, alongside nutrition services, MHPSS, immunization, diagnostics, and structured community outreach.
The Social Worker role is essential to achieving targets, ensuring correct referrals, validating data, and maintaining donor-compliant reporting.

Position Information

  • Duty Station: Akkar & Wadi Khaled (field-based + PHCC coordination)
  • Reporting To: ADJI Project Manager – Health & Nutrition
  • Functional Coordination: Alaaransa/ PHCC Supervisor

Overall Purpose of the Role

The Social Worker ensures that eligible vulnerable individuals are identified, referred, tracked, and correctly counted within the project’s service package, and that community-level data matches PHCC service data and the approved master tracker.

This role directly supports:

  • Achievement of health and nutrition targets
  • Accuracy of monthly tracker, PHENICS data, and invoices
  • Compliance with OCHA-CBPF monitoring, accountability, and safeguarding standards

Core Responsibilities (Strictly Aligned to Project Needs)

1. Community Outreach & Beneficiary Identification

  • Conduct structured outreach in targeted villages of Akkar and Wadi Khaled in line with the project’s geographic coverage.
  • Identify eligible beneficiaries (women, men, boys, girls, PwDs, older persons, PBW, children under five) based on project eligibility criteria.
  • Inform communities about available services at Alaaransa/ PHCC, including:
    • Subsidized PHC consultations
    • Nutrition screening and supplementation
    • MHPSS services
    • Immunization and diagnostics
  • Ensure outreach contributes directly to:
    • 2,000 CHW/community referrals
    • Nutrition and health utilization targets reflected in the master tracker.

2. Case Registration, Referral & Follow-Up

  • Register identified beneficiaries using ADJI-approved outreach and referral forms.
  • Refer beneficiaries to the appropriate PHCC service package (PHC, ANC, NCD, nutrition, MHPSS, diagnostics).
  • Follow up on referrals to confirm:
    • Attendance at Alaaransa/ PHCC
    • Service received matches referral reason
  • Document referral outcomes to support:
    • Indicator achievement
    • Monthly reporting and verification.

3. Direct Support to Project Indicators

The Social Worker’s work must directly feed into the following project indicators:

  • Number of individuals accessing subsidized PHC
  • Number of children and PBW accessing nutrition services
  • Number of caregivers receiving IYCF support
  • Number of PwDs and older persons accessing specialized support
  • Number of beneficiaries reached through outreach and health awareness

All activities must be traceable to the approved master tracker.

4. Data Collection & Accuracy Control

  • Record outreach, referrals, and follow-up data daily.
  • Ensure sex- and age-disaggregated data (W/M/G/B) is correctly captured.
  • Cross-check community referral lists against:
    • PHCC registration logs
    • Service lists provided by the PHCC Supervisor
  • Flag discrepancies immediately and coordinate correction before monthly reporting and invoicing.

5. Support to Monitoring, Reporting & Verification

  • Contribute verified outreach and referral data to:
    • Monthly master tracker updates
    • Monthly narrative inputs
  • Support ADJI monitoring visits by:
    • Providing field evidence
    • Explaining referral pathways and follow-up
  • Ensure all outreach activities have sources of verification (attendance lists, referral forms, visit logs)

6. Accountability to Affected Populations (AAP)

  • Inform beneficiaries about complaint and feedback mechanisms.
  • Support collection of beneficiary satisfaction and feedback surveys when requested.
  • Ensure respectful, inclusive, and confidential engagement with all community members.
  • Escalate complaints or sensitive issues through ADJI’s internal AAP channels.

7. Safeguarding & Ethical Compliance

  • Adhere strictly to ADJI’s PSEA Policy, Code of Conduct, and safeguarding procedures.
  • Ensure safe interaction with women, children, and PwDs during outreach.
  • Immediately report any safeguarding concern through formal channels.
  • Protect beneficiary confidentiality and personal data at all times.

Required Qualifications

  • Bachelor’s degree in Social Work, Psychology, Sociology, Public Health, or related field.

Required Experience

  • Minimum 2 years of experience in humanitarian or community-based projects.
  • Proven experience in outreach, referrals, or case management.
  • Experience working with vulnerable populations in Lebanon is highly preferred.
