Dedicated Focal Point – Monitoring and Reporting (PHC Activities) - 1 year contract

• Support the generation, validation, analysis, and reporting of PHC performance data.
• Strengthen data quality, completeness, and timeliness of PHC reports.
• Generate and monitor key performance indicators (KPIs) aligned with MoPH frameworks.
• Facilitate coordination between PHCCs, implementing partners, and the MoPH.

1. Core Duties & Responsibilities:

Monitoring and Reporting

• Support the generation, consolidation, and timely submission of PHC reports based on agreed indicators and KPIs.
• Generate and monitor KPIs in line with MoPH reporting frameworks.
• Follow up on reporting completeness, timeliness, and consistency across PHCCs and partners.
• Support dissemination and use of reporting tools, templates, and guidance.

Data Validation and Quality Assurance

• Conduct data validation through triangulation of multiple data sources (e.g. partner reports, HIS, PHENICS, administrative records).
• Identify and flag data inconsistencies, gaps, or anomalies.
• Follow up with PHCCs and partners to resolve reporting issues and improve data reliability.
• Participate in data review.
• Conduct periodic field visits to support MoPH oversight of reporting and validation processes.

Coordination and Communication

• Submit bi-weekly and monthly progress reports to MoPH.
• Participate in coordination meetings as required.
• Respond to ad-hoc monitoring and reporting requests under MoPH supervision.
• Tasks may evolve based on programmatic needs.

2. Expected Outputs:

• Timely bi-weekly and monthly PHC monitoring reports submitted to MoPH.
• Validated and consolidated PHC datasets.
• Regularly updated KPIs aligned with MoPH frameworks.
• Improved data quality, consistency, and reporting compliance across PHCCs and partners.
• Documentation of field visits, data validation findings, and follow-up actions.
• Monthly report pertaining to the above activities and attendance schedule
