Assignment: Identify, refer to and follow-up the beneficiaries (mothers, children and pregnant women) in the community. Actively participate in implementation of breastfeeding practices and child care practices and the psycho-social support provided to beneficiaries  Required skills: Sense of confidentiality – Skilled activity leader - Good listening, good communication and adaptation capacity – Good relations with people – Have good empathy and be patient- Fluent English or French highly appreciate. Have a minimum knowledge on infant and young child care practices, including feeding and have the capacity and motivation to learn more in a short space of time Objective 1: Identify target beneficiaries for the “Baby Tents” in the community: Activities: -          Carry out home visits, present the nutrition and mental health activities and explain the reason for the visit. -          Register the families visited and the women and children referred to a nutrition centre. -          Check with the nutritionist and psychologist if the women referred to “Baby Tent” have been admitted in the programme.   Objective 2: Follow-up and home visiting of special cases Activities: -          Collect each day with the psychologist and nutritionist the lists of beneficiaries to visit the day after. -          Give advice according to the situation observed at home. -          Give the result of each home visit to the psychologist and nutritionist. -          Make statistics on the home visits and draft a report of activities   Objective  3 : Work in close collaboration with nutritionist to organize screening and growth monitoring for children who frequent the “Baby Tent” Activities : -          Collaborate with the nutritionist in check the children’s state of health, measure the MUAC and test for oedema. -          Collaborate with the nutritionist to organize growth monitoring sessions -          Inform the parents of the nutritional status of their child. -          Collaborate with the nutritionist to organize screening at community level if necessary   Objective 4: Participate to Baby Tents activities Activities: -          Participate to the organization of the sessions to provide support to the nutritionist and psychologist for management of beneficiaries -          Participate to the welcoming of the beneficiaries into the Baby Tent -          Register newly admitted beneficiaries and perform interviews for them -          Support the nutritionist and psychologist for the planning of activities of the “Baby Tent”   Objective 5: Reporting and coordination Activities -          Participate to creation/revision of project tools -          Realize a weekly and monthly report -          Participate to external reports if needed -          Ensure coordination with partners on the field on request of the supervisors     Due to the emergency context, the social workers can be requested to participate to any other nutrition, mental health and care practices activities who could be identify according to the beneficiaries needs and the humanitarian situation
