The Primary Healthcare Center (PHCC) mental health (MH) nurse is responsible for planning and providing support and nursing care to people who have a range of MH issues. The PHCC MH nurse is part of the PHCC team and works in the PHCC closely with the general practitioner (GP) gatekeeper to implement the management of cases.

 

The PHCC MH nurse answers questions about MH management, keeps track of treatment progress and helps identify side-effects if patients are taking medications. The GP gatekeeper and the PHCC MH nurse work together with the patient. The PHCC MH refers patients for PSS to the MH case worker.

 

Main Responsibilities

 

  1. Keeping accurate medical records and data collection on Phenics
  2. Receiving beneficiaries who are visiting the PHCC/dispensary for mental health services
  3. Screening beneficiaries for mental health disorders
  4.  Participate in beneficiary care Plan in coordination with the mental health team
  5. In coordination with The PHCC receptionist, set the appointment system for the psychiatrist and psychotherapist of the mental health unit
  6. In collaboration with the PHCC/Dispensary pharmacist, participate in the mental health medication management (medication prescription, administration, monitoring of side effects etc.)
  7.  Using the electronic internal referral system, refer beneficiaries to the MH professionals in the PHCC/Dispensary
  8. Coordinate with the PHCC/Dispensary Psychiatrist regarding mental health patients
  9. Participate in the Covid Homecare visits and identify beneficiaries in need of psychosocial support
  10.  Participate in the Peer Educators visits and identify beneficiaries in need of psychosocial support
  11. Participate in the Mobile Medical Unit (MMU) visits, screen and identify beneficiaries with mental health needs
  12.  Assist the triage nurse in the NCD and Depression screening within the PHCC/Dispensary
  13. Conduct Patient Satisfaction Surveys
  14.  Ensure Patient and Caregiver Education on topics related to health problems, mental health disorders, the importance of treatment, specific diet to follow based on each condition, helpful activities etc.

- Registered Nurse-Bachelor of Science in Nursing (RN-BSN), from university or certified technical institutes.
- Specialty in mental health nursing or extensive training in CBT and/or IPT.
- MhGAP training.
- Usage of monitoring and reporting tools such as Google sheets, KOBO toolbox and Phenics health information system.
