Case Manager

The Case Manger will have the following duties and responsibilities:

• Receive the report forms of cases of child protection and of children with specific needs from project staff, investigate through home visits, interviews, classroom observations, compile an initial risk assessment, implement an intervention plan in collaboration with all the involved team with each specific child, monitor the ongoing plan implementation until its closure.
• Have a mapping of the organizations and professionals working in the geographical area given, to whom specific cases should be referred
• Refer identified children to appropriate resources
• Support education assistants and outreach workers with family visits and assessments when requested.
• Report back to M&E team on case management services and support staff in charge of  data compilation
• Submit a monthly report of case management activity and bi-yearly report to be shared with manager of case management
