Medical Social Worker

Job Description: Balsam is looking for a part time social worker who can be part of the interdisciplinary team providing palliative care services to terminally ill patients in their homes in Greater Beirut. The social worker would provide practical and social support to patients and their families. He/she should be interested in this type of end of life care and support. Responsibilities include: -Coordinating patient discharge of patients from medical institutions to his/her home including transportation, arranging for medical supplies and equipment and coordinating any additional services needed. -Conducting family assessments to determine practical and social needs -Ensuring medication and medical equipment delivery and transfer are coordinated -Providing psychosocial support to patients and caregivers - Providing support to family members during the bereavement period Computer Skills: Microsoft Office Please note that owning a car is necessary and work hours are flexible.
