Nurses will be expected to work an average of 10 hours a week and will be compensated accordingly.
Responsibilities include:
- Collecting history of medical problems and previous treatments from the patient and the patient’s family or caregiver.
- Coordinating discharge of patients from medical institutions to his/her home including transportation, arranging for medical supplies and equipment and coordinating additional services needed.
-Conducting a thorough physical and psychosocial assessment and Identifying and prioritizing the patient’s chief complaints
- Developing and implementing a plan of care in conjunction with the patient, family, and healthcare providers in coordination with the patient's and Balsam's physicians
- Providing education to patients, families, and others about palliative care (symptom management…) and patient safety measures
- Documenting care provided in patient medical record and keeping them updated
- Participating in end of life discussions and teaching patients' families about sign and symptoms of imminent death
- Providing support to bereaved families according to Balsam’s bereavement policy and assisting in identifying complicated grief and making referrals accordingly
- Participating in weekly multi-disciplinary meetings and monthly administrative meetings
- Nurses are expected to provide call coverage on nights, weekends and holidays on a rotational basis

Work Experience: A minimum of 2 years of experience in nursing (ICU, Oncology, Pediatrics or ER experience preferred)
