CRS REQUEST FOR QUOTE: MEDICAL AND LIFE INSURANCE

Subject: Request for Quotes

Catholic Relief Services (CRS) is an international non-governmental organization that is registered in Lebanon since 2007, with its offices located in Beirut.

 

CRS is interested in getting bid quotations for Life Insurance for its 29 staff and Medical Insurance for 53 persons (Staff + their families)

To enable Bidders to submit Quotes, please find enclosed the requirements and instructions:

 

Physical offers should be sent to this address: St. Joseph University, Social Sciences Campus, Block B, 6th Fl., Huvelin Street, Monot | Beirut.

 

The physical offer must consist of: one sealed envelope for the insurance plan and another sealed envelope for the financial offer.

Offers should be sent no later than March 6 - 2025 at 16:00 local time.

This letter is not to be construed in any way as an offer to contract with your firm.

All offers received after the indicated time and date will be rejected.

 

 
  1. INSTRUCTIONS TO SUPPLIERS/VENDORS
Medical Coverage:
    1. Description: A Price quotation should be segregated by each category, stating all relevant information and general conditions of your proposal including:

In-Patient: 100% Class A

                  100% Class B and/or Semi-private 

Out-Patient: As per corporate offers

Dr. Visit: As per corporate offers

Prescribed Medicine (including vaccines): As per corporate offers

 

Include GR

Include Continuity

Include New babies to be free of charge until the expiry of the current policy Include Orthosis & Artificial Limbs

Include Radiotherapy, Chemotherapy in case of cancer.

Include Incubator, resuscitation, and Congenital cases for babies. Include Physiotherapy

Include Medical cases caused by passive terrorism &/or passive war risks 15- Include diseases caused by genetic or hereditary conditions.

Include Pandemic Epidemic diseases.

Include Reconstructive surgeries that is medically needed is to be covered but not cosmetic surgery.

Provide staff with medical card for direct billing.

Include Maternity (natural birth and C-Section, Laboratory Tests, echography etc. related to pregnancy)

Include Pre-Existing Conditions / Chronic Diseases Include Dialysis

 

Exclusions: Will-full abortion; birth control and other forms of contraception; cosmetic surgery that are not medically needed.

 

 

 

Life Coverage: $150,000 per person per year, covers must include:

  1. Death
  2. Accidental death benefit
  3. Permanent total disability (Accident & Sick)
  4. Permanent partial disability (Accident &
  5. Passive War (Death/PTD/PPD)

 

    1. Cost of Providing Quotes: Suppliers/Vendors shall bear all costs associated with the preparation and submission of the quote, and CRS will in no case be responsible or liable for those costs, regardless of the conduct or outcome of the solicitation.
  1. Solicitation Documents
    1. Clarification of Solicitation Documents: Suppliers/vendors requiring any clarification on the Request for Quotation may notify CRS only in writing. Clarification may be sought in writing to:

  1. Preparation of Quotes
    1. Language of the Quote: The quote prepared by the supplier/vendor and all correspondence and documents relating to the quote exchanged by the supplier/vendor and CRS shall be in English, or Arabic.

 

    1. Offer Currencies/Offer Prices: All prices shall be quoted in USD, noting that CRS will pay the policies’ costs in fresh US dollar, so all the limits stated above should be calculated on the Sayrafa rate if converted to LBP.

On a side note, for the Life and Medical policy, all the claims shall be paid in fresh US dollar.

Quotes should be segregated by each category of coverage mentioned above (In (Class A, Class B, Class SP, Out, DV, Medicines, etc.) and not only as a total.

 

 
    1. Period of Validity of Quotes: The quote shall remain valid for 365 days after the closing date prescribed by CRS. A quote valid for a shorter period may be rejected as non-responsive.
VI. Opening and Evaluation of Quotes

1. Clarification of Quotes: To assist in the examination, evaluation and comparison of Quotes, CRS may at its discretion ask the Supplier/Vendor for clarification of its Quote.

VII. Preliminary Examination:
  1. Arithmetical errors will be rectified on the following basis: If there is a discrepancy between the unit price and the total price that is obtained by multiplying the unit price and quantity, the unit price shall prevail, and the total price shall be corrected. If the Supplier/Vendor does not accept the correction of errors, its Quote will be rejected. If there is a discrepancy between words and figures the amount in words will prevail.

 

  1. A Quote determined as not substantially responsive will be rejected by CRS and may not subsequently be made responsive by the Supplier/Vendor by correction of the non- conformity.
  2. Evaluation of Quote: Determination of compliance with the RFQ is based on the content of the quote itself without recourse to extrinsic evidence.
