Social Worker

I- GENERAL DESCRIPTION

The main role of the Social Worker (SW) is to support beneficiaries seeking services from Embrace through psychosocial assessments and liaising them to appropriate psycho-social and community-based services that support their mental health needs.

III- MAJOR DUTIES & RESPONSIBILITIES

  • Conducting psychological triage and assessments for those seeking mental health services and aiding in decisions regarding services needed.
  • Conducting face-to face or phone-based case management and following up for existing and new cases in terms of orientation, referral, and other follow-up
  • Collecting, managing, and archiving data pertaining to beneficiaries and community services to establish comprehensive orientation network with a pathway.
  • Revising and/or establishing an efficient and paperless procedure of case- management, within the policies of Embrace.
  • Orienting staff and volunteers at the organization to community resources and case management triages.
  • Engaging with groups present in the humanitarian field to ensure active coordination between different sectors for different service provision.
  • Coordinating family/team meetings as needed and when appropriate.
  • Preparing presentations and/or facilitating workshops.
  • Reporting monthly on the operations of the social work department.
  • Providing education and promoting awareness of mental health and mental illness issues and services.
  • Supporting the mission and vision of Embrace and the National Lifeline.
  • Authorizing financial assistance to help patient meet ongoing medical care expenses and/or forward the social assessment of the patient to the concerned custodian persons to benefit from a certain dedicated fund.
